摘要
目的回顾性分析并探讨颅内动脉瘤的手术技巧和防止术中动脉瘤破裂的措施。方法 24例颅内动脉瘤全部采用翼点入路,动脉瘤夹闭术。结果术前Ⅰ～Ⅲ级23例中有2例轻残,无死亡,Ⅳ级1例死亡。结论术中控制性低血压麻醉,暂时阻断载瘤动脉,充分解剖脑池及采用显微外科技术可以减少术中动脉瘤破裂,提高颅内动脉瘤的手术疗效。选择合适的动脉瘤夹及夹闭动脉瘤颈的方法是成功夹闭动脉瘤的重要环节。要注意变异血管以及深穿支血管的处理,以减少手术后并发症。
Objective Operative skills of intracranial aneurysm and measuers of avoiding the chance of disruption of intracranial aneurysm during the operation were reviewed and discussed in this article. Methods 24 cases of intracranial aneurysm were treated with the clipping technique transpterion approach. Results According to the Hunt - Hess classification, 2 patients were gently disabled and none were dead among the 23 cases with Ⅰ - Ⅲ and 1 patient were dead with Ⅳ .Conclusion Controlle hypotension during the andesthesia , temporarily blocking the artery carrying the aneurysm, dissecting fully the cerebro - cistern and using the microsurgical technique can reduce the chance of disruption of intracranial aneurysm during the operation and enhance the operative effect. Choosing a suitable aneurysm clamp and clipping the aneurysm neck was another important point for the success of the operation. Propely tredeepating the perforating branches and the variational blood vessels can reduce the complications of the operation.
