肩关节镜辅助下小切口肩峰成形及肩袖修补 被引量:14

摘要 目的 探讨肩关节镜辅助下小切口肩袖修补手术的临床应用价值。方法 本组 2 6例患者 ,在关节镜下完成肩峰成形 ,然后在小切口内对肩袖完全破裂的 18例患者行肌腱 骨隧道缝合 ,对部分破裂的 8例患者行肌腱 肌腱缝合。结果 术后随访 2 1~ 4 8个月 (平均 37.5个月 ) ,所有患者均无术后感染及肩关节粘连发生 ,肩关节疼痛 ,特别是夜间痛明显改善 ,手术疗效满意。UCLA评分由术前平均 2 0 .1分改善至术后 31.6分。结论 本术式可确定肩关节面一侧肩袖破裂的部位及深度 ,且不破坏三角肌的连续性。手术效果好 ,患者可较早进行功能煅练 ,功能恢复快。 Objective To appraise the value of arthroscopically assisted mini approach rotator cuff repairs. Methods 26 patients with rotator cuff tears received this procedure. After sub acromial decompression arthroscopically, 18 full thickness tears were sutured by tendon to bone through mini approach, and 8 partial thickness tears were sutured by way of tendon to tendon. Results 26 patients were followed up for 21~48 (average 37.5) months. There were no complication of infection and stiffness. All patients were satisfied with the results. Pain, especially night pain was significantly selieved. UCLA scores were improved from a mean of 20.1 points preoperatively to a mean of 31.6 points postoperatively. Conclusion Location and depth of the articular surface partial thickness rotator cuff tears can be found arthroscopically, with no interraption of the continuity of the deltoid. Active rehabilitation can be started early, and good function can be achieved quickly. This procedure can be recommended extensively, because of its relative simple technique and instrument required.
