摘要
目的:探讨外耳道胆脂瘤临床特点与治疗方法,引起临床医生对该病的关注,提高对本病的认识。方法:对16例外耳道胆脂瘤的临床表现、手术治疗及效果进行了回顾性分析。结果:病变范围小,骨质破坏较轻者5例;虽有骨质破坏,但未进入乳突及上鼓室者6例;侵入上鼓室2例;侵入乳突者3例,并有外耳道后壁-乳突腔瘘形成。结论:外耳道胆脂瘤主要发生在中老年人,它和外耳道栓塞性角化病是两种不同的疾病,治疗的关键是彻底清除胆脂瘤囊袋和死骨。
Objective: investigate the clinical characteristics of external auditory canal cholesteatoma for improve the effect of treatment. Method: Sixteen patients with external auditory canal cholesteatoma were treated from 1988 to 2001 in our department. There were 10 males and 6 females, ranging in age from 15 to 78 years old. CT scaning was using for fefinition the ranging of lesions and the timum management surgery was completed. Results:The lesion ranging in size from a few millimeters to extensive mastoid destruction. The bone distraction was light in 5,heavy bone distraction but no invasive mastoid cavity or tympanic cavity in 6 ,invasion up tympanic cavity in 2,invasion mastoid cavity and formed fistulas between canal and mastoid cavity in 3. All patients were cure for the period of follow-up 5 years. Conclusion: The cholesteatoma of ear canal occurs was infrequently.It is most commonalty see in the elderly. The keratosis obturans and external auditory canal cholesteatoma could be two different clinical and pathologic processes. Smaller lesions can be managed by frequent debridement in the office, larger lesions require a surgical procedure. [Chinese Arch Otolaryngol Head Neck Surg,2003;10(l):6-8
