摘要
目的 :探索在腹腔镜下行带血管蒂回肠段移植再造阴道的可行性。方法 :应用腹腔镜技术完成肠系膜分离、回肠段选取、回肠端端吻合及回肠段下拉移植阴道成形术。结果 :2例手术均获得成功 ,随访 4～ 6月 ,移植肠段成活良好 ,再造阴道无明显挛缩 ,粘膜红润 ,功能和形态恢复基本达到生理要求 ,供区肠功能正常。结论 :腹腔镜用于回肠代阴道手术 ,是创伤小、患者易于接受的阴道成形方法。
Objective:To study the feasibility of total laparoscopic vaginoplasty by transfferring ileal segment flaps.Methods:The abdominal and perineal approaches were performed by laparoscopy, a 15cm segment of the ileum was selected and dissected. The distal of segment was 15cm from the ileocecal junction. The ileum immediately received side to side ananstomosis. Meanwhile, a neovaginal tract was completed and widened laterally. Then the ileum transplant was reversed to reach the perineum through the peritoneal incision at the top of the neovaginal tract. The oral edge of the ileum transplant was sutured to the perineal skin.Results:Good result was obtained in 2 cases of congenital absence of vagina and male to female transsexual was success 4～6 months postoperatively. Conclusions:Laparoscopic ileum colpopoiesis can be used to get satisfactory neovaginal function with mucus and moistness similar to normal vagina, but with no disturbance of bowel function. It has minimal scarring in abdomen and less secondary deformity in perineum with no need for frequent dilation, so it is easy to be accepted by patients.
