摘要
目的 :探讨糖尿病胃轻瘫 (DGP)中医证型与血浆胃动素 (MOT)及体表胃电图 (EGG)改变的关系。方法 :对 38例 3种不同中医证型的DGP患者进行血浆MOT及体表EGG检测 ,并与正常人 2 0例进行对照。结果 :DGP 3种中医证型患者血浆MOT水平明显高于正常对照组 ,以肝胃不和组最高 ,胃阴不足组次之 ,其次是脾胃虚弱组 ;DGP 3种中医证型的EGG餐前、餐后主频率及主功率、正常胃电慢波百分比、胃电节律紊乱百分比与正常对照组比较 ,差异具有显著性 ,以胃动过缓百分比明显增加、正常节律百分比减少为其特征。结论 :DGP的中医证型不同 ,血浆MOT含量及胃电参数不一 ,胃肠动力功能存在明显差异 ,因此MOT及胃电参数可作为DGP中医辨证分型的客观依据之一 。
Objective:To observe the relationship between traditional Chinese medicine (TCM) syndrome type and motilin as well as electrogastrogram in patients with diabetes gastroparesis. Method:Motilin and electrogastrogram were simultaneously measured in 20 cases of healthy subjects (HS)and 38 cases of patients with 3 different kinds of TCM syndrome type and non insulin dependent diabetes gastroparesis (DGP). Result: In DGP patients with 3 different kinds of TCM syndrome type, the level of motilin was higher than that of HS,above the average was patients with incoordination between the liver and stomach,Yin deficiency of stomach, and weakness of the spleen and stomach successively;there was a discrepancy between DGP patients with 3 different kinds of TCM syndrome type and HS in their Fp and Ap level before and after diet, the normal electrogastrogram slow wave percent and the abnormal electrogastrogram percent, which were mainly showed as the increase of delayed gastric peristalsis and the decrease of normal rate. Conclusion: The findings indicate that difference in DGP syndrome type results in variation of serum MOT content, the volume of gastric current and the function of enterogastric motility. Therefore, MOT and gastric current can be regarded as one of the objective bases in classifying DGP according to differentiation of symptoms and signs.
