摘要
目的 比较直肠癌传统Dixon手术和吻合器吻合的手术时间、输血量及并发症。方法 2 0 0 0年 3月至 2 0 0 2年 10月我科共施行直肠癌前切除术 4 3例 ,分为按传统Dixon吻合和吻合器吻合两组。Dixon组 2 8例 ,吻合器组 15例。两组病人的年龄、Dukes分期无明显差别。结果 Dixon组输血量为 (44 5± 4 31)ml,吻合器组为 (45 4± 4 35 )ml(P >0 .0 5 )。Dixon组手术时间为 (2 5 4± 6 5 )min ,吻合器组为 (2 0 6± 4 5 )min(P <0 .0 5 )。肺部感染Dixon组 3例 ,吻合器组 2例。切口感染Dixon组 2例 ,吻合器组 1例。Dixon组未发生吻合口瘘及术后出血 ,吻合器组发生吻合口瘘 3例 ,术后吻合口小量出血 1例。两组无手术死亡。结论 吻合器技术在直肠癌前切除术中的运用能节约手术时间。
Objective To compare the operating time, the volume of blood transfusion and the application in the resection of rectal carcinoma between Dixon and stapling anastomosis. Methods From March 2000 to October 2002, 43 patients underwent the resection of rectal carcinoma. The patients were divided into two groups according to the method of Dixon and stapling anastomosis. There were 28 patients in group A in which Dixon was performed and 15 patients who underwent in group B stapling. There was no significant difference in the age and Dukes stage of rectal carcinoma between the two groups. Results The mean volume of blood transfusion was (445±431) ml in group A and (454±435) ml in group B (P>0.05); the mean operating time was significantly longer in group A than in group B respectively [(254±65)min vs (206±45)min] (P<0.05). Pulmonary infection and incision infection occurred in 3 and 2 patients in group A and 2 and 1 patients in group B respectively. Three patients were comlicated with a leak at the anastomosis and one with postoperative bleeding in group B, but no such complication were found in group A. No patients died in both groups. Conclusions For the resection of rectal carcinoma, the use of the stapler can shorten the operating time.
