摘要
目的比较拜糖平和安慰剂以及拜糖平和二甲双胍的降糖作用,评价拜糖平降糖作用的特点。方法检索1981-01～2001-10的MEDLINE及中国生物医学文献数据库,有关拜糖平降糖作用的随机对照临床试验,且符合入选标准的单项研究入选。计算空腹血糖、餐后血糖、糖化血红蛋白及体重各指标的加权均数差(WMD),并进行Meta分析。结果符合要求的12项随机对照临床试验入选,2项为拜糖平与二甲双胍比较,9项为拜糖平与安慰剂比较,1项为拜糖平、二甲双胍、安慰剂比较。上述临床试验的结果用RevMan3.1软件进行分析。结果显示拜糖平与二甲双胍治疗后空腹血糖、餐后血糖以及糖化血红蛋白WMD分别为-0.52mmol/L(95%CI-0.77～0.26)、-1.18mmol/L(95%CI-1.54～0.81)、-0.31%(95%CI-0.64～0.02),餐后血糖WMD有统计学差异。拜糖平和安慰剂治疗后空腹血糖WMD为-1.18mmol/L(95%CI-1.25～1.11),餐后血糖WMD为-2.69mmol/L(95%CI-0.47～0.44),有统计学差异,糖化血红蛋白WMD为-0.45%(95%CI-0.47～0.44),体重WMD为-0.04kg(95%CI-0.08～0.00),无显著性差异。结论拜糖平与安慰剂相比可有效降低血糖,拜糖平与二甲双胍相比均具有降糖作用,但降低餐后血糖作用更为显著。不良反应较少且轻微。
Objective To get a systematic overview,a meta-analysis of the efficacy in blood glucose co ntrol of Acarbose was performed by comparing Acarbose with placebo,Metformin.M ethods Medline and China Biological Medicine Disk were searched for randomized controlled trials about studying the effect of Acarbose in blood glucose contro l from January,1981to October,2001.Meta-analysis was per-formed cal culating weighted mean difference(WMD)of fasting blood glucose,postprandial b lood glucose,glycosylated hemoglobin,and body weight.Results Twelve randomize d controlled trials were identified,among which two were compared Acarbose wit h Metformin,nine were compared Acarbose with placebo,one was compared Acarbose with placebo and Metformin.The WMD between Acarbose and Metformin after treatm ent for fasting blood glucose,postprandial blood glucose,glycosylated hemoglo bin was-0.52mmol/L(95%CT-0.77～0.26),-1.18mmol/L(9 5%CI-1.54～0.81),-0.31mmol/L(95%CI-0.64～0.02)r espectively,among above12trials postprandial blood glucose WMD had significan t difference after treatment.The WMD between Acarbose and placebo after treatm ent for fasting blood glucose was-1.18mmol/L(95%CI-1.25～1.1 1)and for postprandial blood glucose was-2.69mmol/L (95%CI-2.76 ～2.62),was significant,while there were no significant difference WMD o f glycosylated hemoglobin-0.45%(95%CI-0.47～0.44)and body we ight -0.04kg(95%CI -0.08～0.00)Conclusion Acarbose lowers blo od glucose significantly comparing with placebo,Acarbose and Metformin have an equal effect on blood glucose control.But Acarbose lowers postprandial blood g lucose more significant,and it has less adverse-reaction.
