摘要
目的 :探讨胃癌的X线、CT和B超特征性表现及意义。方法 :186例胃癌均行气钡双重造影 ,其中 63例行CT扫描 ,3 7例行B超检查 ,所有病例均经手术病理证实 ,分析全部病例的影像学表现。结果 :1.X线气钡双重造影检出胃癌 :蕈伞型 65例 ,溃疡型 82例 ,浸润型 3 9例。 2 .CT检出胃癌 :胃壁增厚 3 7例 ,胃内肿块 2 6例 ,胃周脂肪线消失 3 2例 ,胃周及远处淋巴结肿大 2 7例。 3 .B超检出胃癌 :胃壁增厚 17例 ,胃内肿块 11例 ,2 5例显示第 3、4、5层回声线消失、不清或中断 ,13例显示胃周脏器侵润及远处脏器转移 ,12例显示淋巴结转移。结论 :X线气钡双重造影是诊断胃癌的首选检查方法 。
Objective:To analyse the X-ray,CT and ultrasound (US) characteristic manifestatoins of gastric carcinoma.Methods:All 186 cases underwent double-contrast barium enema,63 cases underwent CT,37 cases underwent ultrasonography (USG),All 186 cases were confirmed surgically and pathologically.Results:①186 cases undergoing double-contrast barium enema examination included 65 cases of fungating type,82 cases of ulcerative type and 39 cases of infiltrating type.②63 cases undergoning CT examination,37 cases present gastric wall incrassating,26 cases present tumour in stomach,32 cases present disappearance of fat line around stomach,27 cases present the lymph nodes swelling of contiguity and beyond.③37 cases underwent ultrasound examination,17 cases present gastric wall increasing,11 cases present tumour in stomach cavity,25 cases present the disappearance,clearless or breaking of no.3?4?5 layer echogram,13 cases present metastasis of contiguity and beyond organs,12 cases present swelling of lymph nodes.Conclusion:X-ray gas-barium double contrast examination is the initial method in the diagnosis of carcinoma of stomach,CT and US have the important directing significance in making the curing plan and estimating the prognosis.
