门脉高压性胃病一氧化氮、内皮素、胃泌素的变化及相关性探讨 被引量:2

摘要 目的 探讨门脉高压性胃病 (PHG)患者一氧化氮 (NO)、内皮素 (ET)、胃泌素 (GT)含量的变化在胃粘膜病变中的意义及与临床的相关性。方法 前瞻性随机对PHG 2 9例和慢性胃炎 (CG) 32例病人采用硝酸还原酶法测定空腹血清NO含量 ,用放射免疫分析法测血浆ET、血清GT含量。内镜活检胃粘膜 ,用免疫组织化学法显示诱导型一氧化氮合酶(iNOS)、结构型一氧化氮合酶 (cNOS)和GT活性的变化。结果 ①PHG组血清NO含量、胃粘膜iNOS、cNOS表达均较CG组高 ,差异有显著性 (P <0 .0 5 ) ;②PHG组血浆ET较CG组低 ,差异有显著性 (P <0 .0 5 ) ;NO与ET的相关性其直线相关分析r =-0 .36 0 1,(P >0 .0 5 ) ;③重型PHG血清NO含量较轻型增高 ,ET和GT也较轻型增高 ,但差异均未达显著性 (P >0 .0 5 )。PHG轻重两型与Child Puph肝功能分级无关 ;与食管静脉曲张程度密切相关 ,(P <0 .0 0 0 1) ;④PHG组血清GT较CG组明显低 ,差异有显著性 (P <0 .0 5 ) ,两组胃粘膜GT表达无差异 (P >0 .0 5 )。结论 血NO升高在PHG的发生发展中起重要作用 ,胃粘膜iNOS、cNOS活性增高是血中一氧化氮升高的重要来源。低水平血浆ET对PHT高动力循环起维持作用。血清GT含量减低 ,推测可能由于PHG胃粘膜释放GT发生障碍 ;PHG病人血NO、ET、GT含量的变化会不同?
