阿拓莫兰对体外循环中中性粒细胞核转录因子-κB的干预作用 被引量:1

Intervention of redued glutathin in neutrophil nuclear factor-κB in the process of extracorporeal circulation

摘要 目的 研究阿拓莫兰对临床体外循环 (CPB)患儿中性粒细胞核转录因子 (NF) -κB激活的干预作用。方法 随机抽取年龄 6~ 13岁先心病患儿 2 0例 ,配对分组为对照组 10例、干预组 10例 ,干预组CPB预充液中加入阿拓莫兰 30mg kg,分别于术前 2 4h、CPB开始 2 0min、复温前、CPB结束前、术后 4h、术后 2 4h抽取静脉血 ,分离中性粒细胞 ,提取核蛋白 ,以电泳迁移率实验测定NF -κB活性变化。结果 CPB开始 2 0min后 ,中性粒细胞NF -κB已被激活 ,在复温前达到峰值 ,术后 2 4h逐渐恢复至术前水平 ;干预组NF -κB活性下降 ,差异显著 ( P <0 0 5 )。结论 CPB过程可激活中性粒细胞NF -κB ,在中性粒细胞介导的CPB创伤、炎性反应中起重要作用 ;阿拓莫兰可抑制中性粒细胞NF -κB激活 ,减轻炎性反应。 Objective To study the intervention of redued glutathin in neutrophil nuclear factor-κB in the process of extracorporeal circulation. Methods 20 congenital heart disease children aged from 6 to 12 were selected randomly and were divided into control group (n=10) and intervention group (n=10). Patients of intervention group were added redued glutathin (30 mg/kg) to priming fluid, then phlebotomy was done at 24 h before operation, at 20 min after cardiopulmonary bypass (CPB), before rewarming, the end of CPB, 4 h after operation, 24 h after operation. After separating neutrophil and distilling nuclear protein, we mensurated the activation of nuclear factor-κB by EMSA. Results Neutrophil nuclear factor-κB has been activated at 20 min after CPB, and reached peak on the time of rewarming, and resumed at 24 h after operation in both groups but the intervention group was lower than the control group. Conclusion Neutrophil nuclear factor-κB was activated in the process of extracorpoeal circulation and played an important role in the inflammatory response happening in extracorporeal circulation, which could be relieved by redued glutathin.
