激素替代治疗对绝经后Ⅱ型糖尿病和高血压患者肾脏微血管病变的影响

摘要 目的 :探讨激素替代治疗对绝经后Ⅱ型糖尿病和高血压患者肾脏微血管病变的影响。方法 :36例患有Ⅱ型糖尿病和高血压的绝经后妇女随机分为治疗组和安慰剂组各 18例 ,采用双盲法予口服克龄蒙或安慰剂 1片 /日 ,共 4 5个周期 ,测量用药前后各参数值并进行比较及线性相关分析。结果 :治疗组用药前后 2 4小时尿蛋白定量由 ( 0 4 45± 0 0 36 )g降到 ( 0 36 1± 0 0 32 ) g(P <0 0 1) ,内生肌酐清除率由 ( 92 0± 5 2 )ml/min增到 ( 99 1± 4 8)ml/min (P <0 0 5) ,空腹血糖由 ( 7 0 2± 0 4 3)mmol/L降到 ( 6 55± 0 31)mmol/L(P <0 0 5) ,血清总胆固醇由 ( 6 6 6±0 2 8)mmol/L降到 ( 5 71± 0 71)mmol/L(P <0 0 1) ,血压无明显改变。 2 4小时尿蛋白定量和内生肌酐清除率与其他参数间无显著相关性。安慰剂组用药前后各项指标无明显改变。结论 Objective:To investigate the effect of hormone replacement therapy(HRT)on renal microvascular lesion in postmenopausal women with type Ⅱ diabetes mellitus and hypertension.Methods:Thirty-six postmenopausal women with type Ⅱ diabetes mellitus and hypertension were assigned to 2 groups randomly,group of therapy and group of placebo,18 cases in each group.All participants received combined therapy of estrogen and progesterone or placebo as control double-blindly for 4\^5 monthly cycles.We measured the parameters before and after starting HRT 4\^5 months respectively.Results:In the group of therapy,comparing the baseline values,24-hour fixed quantity of urinary protein was reduced from (0 445±0 036)g to(0 361±0 032)g(P<0 01),endogenous creatinine clearance rate was increased from(92 0±5 2)ml/min to(99 1±4 8)ml/min (P<0 05),fasting plasma glucose also reduced from(7 02±0 43)mmol/L to(6 55±0 31)mmol/L(P<0 05),as did serum total cholesterol from(6 66±0 28)mmol/L to(5 71±0 17)mmol/L(P<0 01),blood pressure did not change significantly.Linear regression analysis showed no significant correlation between the individual changes in 24-hour fixed quantity of urinary protein or endogenous creatinine clearance rate and the individual changes in the other parameters.All above parameters had no statistically significant difference versus baseline after 4 5 treatment months in the group of placebo.Conclusions:The HRT had an improvement role on renal microvascular lesion in postmenopausal women with type Ⅱ diabetes mellitus and hypertension.
