摘要
目的 了解低分子肝素 (lowmolecularweightheparin,LMWH)抗凝对尿毒症维持性血液透析患者的脂质代谢的影响 .方法 对 46例透析患者随机分成普通肝素 (UFH)组和LMWH组 ,比较治疗前 ,治疗后 2个月、4个月、6个月的血甘油三酯 (TG)、总胆固醇 (TC)、低密度脂蛋白胆固醇 (LDL -C)、高密度脂蛋白胆固醇 (HDL -C)、载脂蛋白A1(Apo -A1)、载脂蛋白B(Apo-B) .结果 LMWH组治疗后随时间延长 ,TG、TC、LDL -C、Apo -B逐渐下降 ,HDL -C、Apo -A1逐渐升高 ,而UFH组无类似变化 ,两组差异有显著性意义 (p <0 .0 5 ) .结论 在维持性血液透析中以LMWH代替UFH抗凝可一定程度上改善透析患者的脂质代谢障碍 。
Objective To find out the effect of low molecular weight heparin on lipometabolism in patients with end-stage renal disease treated by hemodialysis. Methods 46 dialysis patients were randomly divided into two groups :UFH(unfractionated heparin) group ( n=23) and LMWH (low molecular weight heparin) group (n=23),The level of serum TG、TC、HDL-C、LDL-C、Apo-A1、Apo-B were measured at month 0,2,4 and 6. Results As dialysis time went by,in LMWH group,TG、TC、LDL-C、Apo-B concentration decreased,in the same time,HDL-C、apo-A1 concentration increased,significantly different in contrast to UFH group,it may help to lower the risk of cardiac event. Conclusions The long term use of LMWH instead of conventional heparin of anticoagulation during dialysis has beneficial effect in patients with dyslipidoses.
