电子束CT三维重建在髋臼骨折诊断及治疗中的应用

摘要 目的 :探讨和评价电子束CT(EBCT)三维重建技术在髋臼骨折中的诊断和治疗价值。 方法 :对 2 3例髋臼骨折患者行EBCT平扫和三维图像重建处理。对髋臼骨折进行分类 ,选择合适的治疗方案。 结果 :EBCT三维重建能确切显示髋臼骨折及其类型 ,依据骨折类型及损伤的严重程度 ,确定治疗方案。 2 3例患者中 ,14例行手术治疗 ,9例行保守治疗 ,平均随访 1.3年 ,髋关节功能恢复优良率为 82 .6 %。 结论 :EBCT平扫及三维重建可以确定髋臼骨折的类型及损伤程度 ,根据图像结果选择合适的治疗方案。在行手术治疗时 ,能指导选择最佳的手术入路及复位固定方法 。 Objectives:To assess the clinical value of three dimensional imaging of electron beam CT(EBCT) in the diagnosis and treatment of acetabular fractures. Methods:Twenty three cases of acetabular fractures were examined under EBCT scan and 3D reconstruction. According to the images, 23 cases were classified into groups for the selection of therapies. Results:Among all these patients, 14 cases were treated by open reduction, and 9 treated by conservative therapy. All patients were followed for an average of 14 months. Conclusions: EBCT scan and 3D reconstruction clearly demonstrates the types and the severity of acetabular fractures. EBCT provides an exact image, which guides the suitable approach in the operation and the methods of open reduction, which is important in the clinical experience.
