摘要
[目的 ]应用吻合血管的股前外侧皮瓣治疗小腿及足跟足底大面积软组织缺损伴骨外露 ,并探讨其优点 .[方法 ]对 7例不宜应用局部皮瓣转移修复的小腿及足跟足底广泛软组织缺损伴骨外露的创面 ,采用吻合血管的股前外侧皮瓣游离移植修复创面 ,并带部分肌瓣、髂胫束以及股外侧皮神经 ,骨折分别行内固定或外固定 .[结果 ]应用股前外侧皮瓣修复 4例小腿软组织缺损及 3例足底软组织缺损伴骨外露 ,皮瓣全部成活 ,创面得以修复 ,大部分足底皮肤恢复感觉 ,患肢功能恢复良好 .[结论 ]股前外侧皮瓣具有皮瓣隐蔽、可带肌肉及股外侧皮神经等优点 。
OBJECTIVETo treat widespread defects of soft tissue in crus and sole of foot with exposed bone by using free grafting of anterior lateral femoral skin flap and to report the effect and advantages.METHODS7 cases of wound face with widespread defects of soft tissue in crus and sole of foot with exposed bone that could not repaired with local skin flap were repaired by using the free grafting of anterior lateral femoral skin flap with vascular anastomosis, part muscle valve, iliotibial tract and lateral femoral cutaneous nerve, and the fractures were performed with internal and external fixation.RESULTSAfter repaired, 4 cases of widespread defect of soft tissues in crus and 3 cases of widespread defects of soft tissue on sole of foot by using anterior lateral femoral skin flap, and all the skin flaps grew well, and the wound faces were finely repaired, and the sense of most skin of sole of foot had recovered and the function of affected extremities was recovered well.CONCLUSIONThe anterior lateral femoral skin flap has the advantages of that the flap was secluding and could be taken with lateral femoral cutaneous nerve, therefore it is good for repair of widespread defects of traumatic soft tissues in crus and sole of foot with exposed bone.
