摘要
目的 探讨高血压性脑出血术后死亡原因。方法 对我院 5年来采用常规手术治疗高血压性脑出血 186例进行回顾性分析。结果 6 0岁以上手术死亡率 33 7% ,发病 2h内即呈中度以上昏迷手术死亡率79 3% ,发病后 6h内手术死亡率 17 2 % ,7～ 2 4h内手术死亡率 30 8% ,发病 2 4h后才进行手术死亡率 5 5 6 %。在4 6例死亡病例中 ,出血量在 6 0ml以上占 78 3% ,有 73 9%并发严重肺部感染和低氧血症 ,有 5 4 3%出现上消化道出血。结论 发病年龄越大 ,发病距脑疝症状时间越长 ,手术距发病、脑疝症状时间越长 ,血肿量越大 ,术后出现严重并发症者 。
Objective To discuss the causes of death on surgery treating hypertension cerebral hemorrhage.Methods:186 cases hypertension cerebral hemorrhage patients who had been treated by routine operation were retrospective analysised.Results the operative mortality was 33.7% who were over 60 years old ,and the mortality was 79.3% in which moderate or more coma within two hour onset,and the operative mortality was 17.2% within 6h onset,and the operative mortality was 30.8% within 7-24h onset,and the operative mortality was 55.6% after 24h onset.Of 46 cases demise patients,78.3% amount of bleeding more than 60ml,73.9% accompanying by severe pulmonary infection and hypoxemia,54.3% accompanying by upper gastrointestinal bleeding.Conclusions The more old the patient onset and the more long time between cerebral hemorrhage and onset,and the more long between surgery and onset or cerebral hernia,and the more volume hematoma,as well as severe complication after surgery surgical skill,the higher mortality.
