摘要 目的 :推荐一种快速、敏感、特异的可以早期诊断肾移植受者并发巨细胞病毒 (Cytomegalovirus,CMV)感染的检测技术。方法 :使用实时在线荧光定量PCR(Real-TimePCR)对39名肾移植受者的标本进行CMV— -DNA检测。结果 :39名肾移植受者有17名检出CMV -DNA ,阳性检出率为43 6 % ;116份血浆标本中检出5例阳性 ,中位数 (Md)为5 18×102 拷贝/ml,116份尿液标本中检出13例阳性 ,Md为6 93×102 拷贝/ml,P<0 05 ,未检出同一患者血浆和尿液标本同时阳性的病例。结论 :CMV是肾移植受者中常见的感染 ;尿液与血浆中CMV -DNA检出率的差异具有统计学意义 ;Real-TimePCR是监测肾移植受者CMV感染进程和监测抗病毒药物疗效的有效工具。 Objective:To recommend a rapid,specific and sensitive diagnostic method which is essential for early detection of CMV infection in patients after kidney transplantation.Methods:A quantitative real-time PCR assay was used to measure CMV DNA load in samples from 39 renal transplant recipients.Results:In this study 17(43.6%)patients were CMV DNA positive during the montioring period.The positive cases of 116 plasma samples and 116 urine samples were 5(Median,Md=5.18×102copies/ml)and 13(Md=6.93×102copies/ml)respectively(P<0 05).Conclusion:It is concluded that CMV infection is common in patients receiving renal transplants,and the real-time PCR assay for the quantification of CMV DNA may provide a useful tool that can be used to monitor disease progression,evaluate therapeutic regimens,and recognize viral resistance and drug failure.
