非肥胖2型糖尿病胰岛素抵抗与脂代谢的关系

摘要 目的 研究非肥胖2型糖尿病患者胰岛素抵抗与血脂代谢的关系。方法 测定72例非肥胖糖尿病患者及60例非肥胖正常人空腹血糖(FPG)、糖化血红蛋白(HbA1c)、空腹胰岛素(FINS)、体重指数(BMI)及脂代谢指标,并进行相关性分析。结果 ①72例非肥胖糖尿病患者FPG、HbA1c、FINS、胆固醇(CHO)、甘油三酯(TG)、低密度脂蛋白(IDL)、极低密度脂蛋白(VLDL)及胰岛素抵抗指数(HOMA-IR)明显高于正常对照组。②Pearson相关分析表明HO-MA-IR与CHO、TC、LDL.呈正相关。③逐步回归分析发现以HONA-IR为因变量,CHO、TG、LDL为自变量,TG对HO-MA-IR有影响。结论高TG是非肥胖2型糖尿病胰岛素抵抗的特点,他升高可作为非肥胖2型糖尿病胰岛素抵抗的独立因素。 Objective To study the relation between insulin resistance and lipid metabolism in nonobese type 2 diabetes mellitus (DM). Methods Fasting plasma glucose (FPG), HbA]c, fasting insulin (FINS), body mass index (BMI) and plasma lipids were determined in 72 cases of nonobese type 2 diabetes and 60 cases of nonobese health controls. Results (1)The levels of FPG, HbAlo, FINS, cholesterol (CHO), triglyceride (TG), low density lipoprotein (LDL), very low density lipoprotein (VLDL) and homeostasis model assessment-insulin resistance (HOMA-IR) in the nonobese type 2 DM were higher than those in the control group. (2)Pearson correlation analysis indicated that there existed positive correlation among HOMA-IR and CHO,TG,LDL.(3)Step-wise regression analysis indicated that HOMA-IR was independently associated with TG. Conclusion Nonobese type 2 diabetic patients with insulin resistance are characterized by high TG levels, and TG could be regarded as independent factor for insulin resistance.
