摘要
目的 总结内侧型蝶骨嵴脑膜瘤的显微手术治疗体会及分析存在问题。方法 回顾性分析26例内侧型蝶骨嵴脑膜瘤的显微手术治疗情况。结果 肿瘤全切17例,近全切5例,部分切除4例。术后死亡1例,3例术后视力障碍较术前加重,1例发生肢体偏瘫。15例随访3月至5年,全切的10例肿瘤无复发;近全切及部分切除的5例中2例肿瘤无变化,有3例肿瘤增大。结论 内侧型蝶骨嵴脑膜瘤的治疗至今仍是个难题,手术治疗应在保护神经功能的前提下最大限度的切除肿瘤,放疗在内侧型蝶骨嵴脑膜瘤的治疗中起重要作用。
Objective To summarize the experience of the microsurgical treatment of medial sphenoid ridge meningiomas (MSRMs). Methods The clinical data of 26 patients with MSRMs were analyzed retrospectively. Results Of 26 cases of MSRMs, 17 were totally removed, 5 subtotally and 4 partially. One patient died postoperatively. The visual disturbance occurred in 3 patients and hemiplegia in 1. In 15 patients followed up for 3 months to 5 years, the tumors did not recur in 10 patients received total resection of the tumors, the residual tumors did not change in 2 received subtotal or partial resection and the residual tumors enlarged in 3 received subtotal or partial resection of the tumors. Conclusion The MSRMs should be resected as totally as possible on the base of preservation of the neurological function and receive radiotherapy after the operation.
