摘要
目的 :观察妊娠高血压综合征 (妊高征 )患者的血管内皮损伤、血小板活化状态、抗凝及纤溶系统部分功能指标的变化 ,探讨其临床意义。方法 :检测正常非孕妇女、正常晚期妊娠妇女各20例和62例妊高征患者的血管性血友病因子 (vWF)、血小板α_颗粒膜蛋白 (GMP_140)、抗凝血酶_III(AT_III)、蛋白C系统筛选 (ProCGlobal)、纤溶酶原 (PLG)、组织纤溶酶原激活物 (t_PA)及其抑制物 (PAI)和D_二聚体 (D_Dimer)的含量。结果 :与正常非孕组及正常晚期妊娠组比较 ,妊高征组vWF、GMP_140水平均显著增高 ,并随病情加重而呈增高趋势 (P<0.01) ,而AT_III、ProC活性均显著下降 (P<0.01) ,病情越重下降越明显 ;妊高征组PLG、D_Dimer、PAI均显著增高 (P<0.01) ,中重度患者尤其明显 ,而t_PA无显著性改变 (P>0.05)。结论 :正常晚期妊娠妇女血液处于高凝状态 ,而妊高征患者存在明显血栓前状态 ,因此产前测定凝血和纤溶功能指标 ,对监测病情、指导治疗。
To study changes of the hemorrheology,coagulation,and fibrinolytic system in patients with pregnancy_induced hypertension syndrome(PIH).Methods:To examine the platelet alphagranule membraneprotein(GMP_140),vonwillebrand factor(vWF),antithrombin III activity(AT_III),plasminogen activity(PLG),the activity of tissue_type plasminogen activator(t_PA),and plasminogen activator inhibitor(PAI) in 20 non_pregnancy,20 normal pregnancy women,and 62 patients with PIH.Results:vWF,GMP_140,PLG,D_Dimer,and PAI were significantly higher in PIH group than those in control group.The activities of AT_III and Proc in HIP group were significantly lower than those in control group(P<0.01).There was no significant difference in t_PA between HIP and control group(P>0.05).Conclusion:There are elevated platelet activation,fibrinolytic activity,and injury of vascular endothelium in patients with PIH.Antenatal examination of coagulation function has a high value in prevention and treatment of HIP.
