摘要
目的探讨2型糖尿病早期肾损害时近曲小管功能情况。方法 同期对33例2型糖尿病患者和20例正常人测定晨尿Alb/Cr比值和测定晨尿α1-MG、TRU、IgG浓度。结果 (1)正常白蛋白尿组(DM-Ⅰ)与对照组(NC组)比较:尿α1-MG显著升高(P<0.001),而尿Alb/Cr、TRU、IgG无显著的差异。(2)微量白蛋白尿组(DM-Ⅱ)与NC组比较:尿Alb/Cr、α1-MG、TRU、IgG均显著升高(P<0.001)。(3)DM-Ⅱ组与DM-Ⅰ组比较:尿Alb/Cr、α1-MG、TRU、IgG均显著升高(α1-MG,P<0.01,其余P<0.001)。结论 2型糖尿病肾脏早期损害,除肾小球病变之外,也存在近曲小管病变,甚至早于肾小球病变。
Objective To investigate the function of proximal convoluted tubule in early renal damage stage in type 2 diabetes mellitus.Methods Urinary albumin/creatine (Alb/Cr)α1-microglobulin(α1-MC), transferin(TRU) and immunoglobulin G(Ig G) were measured in 33 diabetes patients and 20 normal people . Results 1) Among the patients with normal albuminuria, urinary on -MG increased significantly compared with the control group, while there was no significance in urinary Alb/Cr, TRU, IgG between the two groups.2) Among the patients with micro-albuminuria, urinary Alb/Cr, α1-MG, TRU and IgG increased significantly compared with the control group.3) Between the patients with micro-albuminuria and those with normal albuminuria, urinary Alb/Cr, α1-MG, TRU, IgG increased significantly in the group with micro-albuminuria.Conclusions The tubulointerstitium damage was common in type 2 diabetes except glomeralosolerosis, even earlier than glomerulus damage.
