感染性休克心肌细胞损害与心功能不全的关系 被引量:8

The relationship between myocardial cell injury and cardiac dysfunction in septic shock

摘要 目的 观察感染性休克心肌细胞损伤情况 ,探讨其与心功能不全的关系。方法  8只杂种犬于 2 0分钟内持续静脉注入内毒素 2 5 0 μg/kg ,建立感染性休克模型。插肺动脉导管测定休克前和休克后 1小时左心室每搏作功指数 (LVSWI) ;同时用抗心肌肌钙蛋白I单抗酶联免疫法测定血清心肌钙蛋白I(cTnI)。结果  8只感染性休克犬休克 1小时后的LVSWI均明显低于休克前 ,其中6只的LVSWI<4 0g·m·m-2 。休克 1小时 ,4只犬的血清cTnI>0 6 μg/L[cTnI(1 96± 0 73) μg/L];这 4只犬的LVSWI明显低于其余 4只犬 (P <0 0 5 )。结论 感染性休克犬心肌细胞损伤常见 。 Objective To evaluate the correlation between myocardial cell injury and cardiac dysfunction in septic shock.Methods Eight dogs were received an infusion of 250μg/kg Escherichia coli endotoxin and left ventricular stroke work index(LVSWI)was measured at baseline,1 hour after an infusion LPS by Swan Ganz catheter.Serum cardiac troponin I(cTnI) was measured at the same time with a double monoclonal'sandwich'enzyme immunoassay.Results Myocardial dysfunction was found in 6 of the 8 dogs with septic shock (LVSWI<40g·m·m -2 ).The cTnI was significantly higher in 4 septic dogs with myocardial cell injury than that in another 4 dogs without[(1 96±0 73)vs(0 29±0 07)ug/L,( P< 0 05)],and the opposite change was LVSWI[(19 38±2 46)vs (32 14±9 33)g·m·m -2 ,( P< 0 05)].Conclusion Myocardial cell injury appears to be common in dogs with septic shock and correlates with cardiac dysfunction. [
