摘要
目的分析三维CT重建的肩峰形态参数与肩袖撕裂的相关性。方法选择2022年1月至2024年12月遵化市人民医院骨科收治的106例肩袖损伤患者开展回顾性分析,根据是否肩袖撕裂分为肩袖撕裂组(n=58)和肩袖完整组(n=48)。所有患者均行肩关节三维CT扫描,重建后测量肩峰倾斜角、肩峰指数、临界肩角、肩峰外侧角、肩肱距离及肩峰厚度。比较两组间参数水平,应用二元Logistic回归分析对肩袖撕裂的危险因素进行分析,应用双变量相关性分析对各个参数与肩袖撕裂的相关性进行分析。结果肩袖撕裂组患者的肩峰倾斜角、肩峰指数、临界肩角、肩峰厚度分别为(33.08±5.12)°、0.73±0.05、(35.16±3.98)°、(8.53±1.12)mm,均明显高于肩袖完整组[(27.15±4.75)°、0.68±0.07、(31.82±3.34)°、(8.02±1.04)mm],肩峰外侧角、肩肱距离分别为(71.32±6.55)°、(6.62±1.24)mm,均明显低于肩袖完整组[(75.29±7.11)°、(7.78±2.23)mm],差异均有统计学意义(P<0.05)。二元Logistic回归分析结果显示,肩峰倾斜角、肩峰指数、临界肩角、肩峰外侧角、肩肱距离均是肩袖撕裂的危险因素(P<0.05),肩峰厚度是肩袖撕裂的保护因素(P<0.05)。双变量相关性分析结果显示,肩峰倾斜角、肩峰指数、临界肩角、肩峰厚度与肩袖撕裂均呈显著正相关性(P<0.05),肩峰外侧角、肩肱距离与肩袖撕裂均呈显著负相关性(P<0.05)。结论肩峰形态参数是肩袖撕裂的重要形态学危险因素,三维CT重建可精准量化评估肩峰形态,为临床诊疗提供影像学依据。
Objective To analyze the correlation between acromion morphological parameters and rotator cuff tear based on three-dimensional CT reconstruction.Methods A retrospective analysis was performed on 106 patients with rotator cuff injury admitted to the Department of Orthopedics,Zunhua People's Hospital from January 2022 to December 2024.According to whether rotator cuff tear occurred,they were divided into the rotator cuff tear group(n=58)and the intact rotator cuff group(n=48).All patients underwent three-dimensional CT scans of the shoulder joint.After reconstruction,the acromial inclination angle,acromial index,critical shoulder angle,lateral acromial angle,shoulder-brachial distance,and acromial thickness were measured.The parameter differences between the two groups were compared.Binary Logistic regression analysis was applied to analyze the risk factors of rotator cuff tear,and bivariate correlation analysis was used to analyze the correlation between each parameter and rotator cuff tear.Results The acromioclavicular tilt angle,acromioclavicular index,critical acromioclavicular angle,and acromioclavicular thickness in the rotator cuff tear group were(33.08±5.12)°,0.73±0.05,(35.16±3.98)°,and(8.53±1.12)mm,respectively,which were significantly higher than those in the intact rotator cuff group[(27.15±4.75)°,0.68±0.07,(31.82±3.34)°,and(8.02±1.04)mm],the lateral acromioclavicular angle and shoulder-brachial distance were(71.32±6.55)°and(6.62±1.24)mm,respectively,which were significantly lower than those in the intact rotator cuff group[(75.29±7.11)°,(7.78±2.23)mm],and the differences were statistically significant(P<0.05).The results of binary Logistic regression analysis showed that acromion tilt angle,acromion index,critical acromion angle,lateral acromion angle and shoulder-brachial distance were all risk factors for rotator cuff tear(P<0.05),and acromion thickness was a protective factor for rotator cuff tear(P<0.05).The results of bivariate analysis showed that acromion tilt angle,acromion index,critical acromion angle and acromion thickness were significantly positively correlated with rotator cuff tear(P<0.05),while acromion lateral angle and shoulder-brachial distance were significantly negatively correlated with rotator cuff tear(P<0.05).Conclusion The morphological parameters of the acromion are important morphological risk factors for rotator cuff tear.Three-dimensional CT reconstruction can accurately and quantitatively evaluate the morphology of the acromion,providing imaging evidence for clinical diagnosis and treatment.
