摘要
目的 探讨安宫牛黄丸联合高压氧(HBO)治疗高血压脑出血(HICH)疗效。方法 纳入2023年6月至2024年12月HICH患者90例,随机分为对照组(45例,脱落2例)和观察组(45例,脱落1例)。对照组给予HBO治疗,观察组给予HBO联合安宫牛黄丸治疗。两组患者均治疗2周。比较治疗前、后两组神经功能(NIHSS评分)、中医证候积分、日常生活能力(MBI评分)、炎症因子(IL-6、TNF-α)及脑损伤标志物(S100-β、NSE)水平、感染发生率,并评价安全性。结果 治疗后,两组患者NIHSS评分、中医证候积分、IL-6、TNF-α及S100-β、NSE均降低(P<0.05);MBI评分提高(P<0.05),且观察组优于对照组(P<0.05)。观察组感染发生率低于对照组(P<0.05)。两组治疗期间均未发生严重不良反应。结论 安宫牛黄丸联合HBO治疗在HICH早期康复中有较好疗效,且安全性良好。
Objective To explore the efficacy of Angong Niuhuang Pills combined with hyperbaric oxygen(HBO)therapy in the treatment of hypertensive intracerebral hemorrhage(HICH).Methods From June 2023 to December 2024,90 patients with HICH were randomly assigned tc a control group(45 cases,with 2 dropouts)and an observation group(45 cases,with 1 dropout).The control group received HBO therapy,while the observation group received HBO therapy combined with Angong Niuhuang Pills.Both groups were treated for 2 weeks.Neurological function(NIHSS score),traditional Chinese medicine(TCM)syndrome scores,activities of daily living(MBI score),levels of inflammatory factors(IL−6,TNF-α),brain injury markers(S100β,NSE),infection incidence,and safety were compared before and after treatment.Results After treatment,both groups showed reductions in NIHSS scores,TCM syndrome scores,IL−6,TNF−α,S100−β,and NSE levels(P<0.05).MBI scores improved(P<0.05),with the observation group demonstrating superior outcomes compared to the control group(P<0.05).The incidence of infection in the observation group was lower than that in the control group(P<0.05).No severe adverse reactions occurred during treatment in either group.Conclusion The combination of Angong Niuhuang Pills and HBO therapy demonstrates good efficacy and safety in the early rehabilitation of patients with HICH.
