基于临床便利性的GBS预防青霉素剂量选择:240万单位与250万单位的非劣效性观察

摘要 目的 评估在预防B族链球菌(GBS)母婴传播中,使用240万单位与250万单位青霉素G进行产时预防的临床效果与安全性差异。方法 纳入2019年1月至2024年12月期间医院收治的422例GBS阳性孕妇资料。根据实际给药剂量分为修正剂量组(240万单位,n=297)与标准剂量组(250万单位,n=125)。比较两组产妇产后炎症指标及新生儿炎症指标。结果 两组产妇产后的白细胞计数、中性粒细胞计数差异均无统计学意义(均P>0.05)。两组新生儿出生后的白细胞计数、中性粒细胞计数及C反应蛋白水平对比,差异也无统计学意义(均P>0.05)。随访期间,两组新生儿均未发生早发型GBS疾病(GBS-EOD)。结论 在本研究回顾性分析中,240万单位与250万单位青霉素G预防方案,在母婴炎症反应指标及短期结局上未见显著差异。240万单位青霉素G预防方案在实际操作中可能更具便利性,但其等效性及临床应用价值仍需前瞻性研究进一步验证。 Objective To evaluate the clinical efficacy and safety differences between using 2.4 million units and 2.5 million units of penicillin G for intrapartum prophylaxis in preventing maternal—to-neonatal transmission of Group B Streptococcus(GBS).Methods Data from 422 GBS-positive pregnant women admitted to the hospital between January 2019 and December 2024 were included.Based on the actual administered dose,they were divided into a modified dose group(2.4 million units,n=297)and a standard dose group(2.5 million units,n=125).Postpartum maternal inflammatory markers and neonatal inflammatory markers were compared between the two groups.Results There were no statistically significant differences in postpartum white blood cell count or neutrophil count between the two maternal groups(all P>0.05).Similarly,no statistically significant differences were found between the two neonatal groups regarding white blood cell count,neutrophil count,or C-reactive protein levels after birth(all P>0.05).During the follow-up period,no cases of early-onset GBS disease(GBS-EOD)occurred in either neonatal group.Conclusion In this retrospective analysis,no significant differences were observed between the 2.4 million-unit and 2.5 million-unit penicillin G prophylaxis regimens in terms of maternal and neonatal inflammatory response markers and short-term outcomes.The 2.4 million-unit penicillin G regimen may offer greater practicality in clinical application,but its equivalence and clinical value require further validation through prospective studies.
