摘要
目的:探讨电针针刺合谷穴和内关穴在糖尿病视网膜病变患者全视网膜光凝术(pan–retinal photocoagulation,PRP)中的镇痛效果。方法:共纳入37例糖尿病视网膜病变患者,分为电针组和对照组,两组患者分别接受两次治疗。对照组在两次治疗中均不接受任何干预。电针组分为两个亚组,仅在其中一次PRP时接受电针干预,另一次则不予干预:电针1组患者仅在第一次PRP治疗时接受电针治疗,电针2组患者仅在第二次PRP治疗时接受电针治疗。电针仪波形为连续波,频率1 Hz,强度3~5级,作用时间为从PRP开始前10分钟至PRP结束。穴位选择合谷穴和内关穴,疼痛强度测量工具为视觉模拟评分量表(visual analogue scale,VAS)。结果:在第一次PRP治疗中,电针1组与对照组的疼痛评分无显著差异(P=0.190)。在第二次PRP治疗中,电针2组的疼痛评分显著低于对照组(P=0.003)。将同一电针组内两次PRP的疼痛评分进行比较,接受电针治疗后的疼痛评分显著低于未接受干预的评分(电针1组:P=0.001;电针2组:P=0.025)。对照组在两次PRP中的疼痛评分无显著差异(P=0.060)。结论:电针针刺合谷穴和内关穴可能有助于缓解糖尿病视网膜病变患者PRP治疗中的疼痛。
Objective:To evaluate the analgesic effect of electroacupuncture(EA)at Hegu(LI4)and Neiguan(PC6)in patients with diabetic retinopathy(DR)who are undergoing pan-retinal photocoagulation(PRP).Methods:A total of 37 patients were recruited.The PRP was performed in two sessions.Patients with DR were randomly divided into EA group and control group,with both groups receiving two treatment sessions.The control group received no intervention during the procedure.The acupuncture group was further divided into two subgroups,receiving EA only during one of the two sessions,while the other session received no intervention:the first sub group(EA1)received EA only during the first PRPsession,while the second sub group(EA2)received EA only during the second PRPsession.EA was applied at the LI4and PC6acupoints bilaterally.The EA device was set to a continuous wave at a frequency of 1 Hz and an intensity of 3-5,applied from 10 minutes before PRP until the end.Pain intensity was assessed using the Visual Analogue Scale(VAS).Results:During the first PRPsession,there was no significant difference in pain scores between the EA1 and the control groups(P=0.19).During the second session,the scores in the EA2 group were significantly lower than that in the control group(P=0.003).In both EA groups,the pain scores following EA were significantly lower than during ho-intervention sessions(EA1:P=0.001;EA2:P=0.025).Within the control group,no statistically significant difference was observed in pain scores between the two sessions(P=0.060).Conclusion:Electroacupuncture at LI4 and PC6 may alleviate the pain in patients with DR undergoing PRP.
