移植肾超声定位显微成像参数与其功能及病理损伤的相关性

摘要 目的分析移植肾超声定位显微成像(ULM)参数与其功能及病理损伤的相关性。方法回顾性纳入23例肾移植术后患者,根据移植肾功能恢复状态和病理结果将其分为功能稳定组(A组,n=6)、急性损伤组(B组,n=7)及慢性功能不全组(C组,n=10);比较组间常规超声及ULM参数,以Pearson相关分析观察ULM参数与估算肾小球滤过率(eGFR)及病理Remuzzi评分的相关性。结果3组间移植肾微血管密度占比、平均密度、血容量、平均流速及灌注指数差异均有统计学意义(P均<0.01),而常规超声参数及微血管复杂度、最大密度、最大流速差异均无统计学意义(P均>0.05)。两两比较,B组微血管密度占比、平均流速及灌注指数均低于A组(P均<0.05),C组微血管密度占比、平均密度、血容量、平均流速及灌注指数均低于A、B组(P均<0.05)。微血管密度占比、平均流速和灌注指数与eGFR均呈低度正相关(r=0.440、0.420、0.484,P均<0.05)。微血管平均流速与Remuzzi评分呈高度负相关(r=-0.864,P=0.012)。结论移植肾ULM参数与其功能及病理损伤存在相关性。 Objective To explore the correlations of ultrasound localization microscopy(ULM)parameters,function and pathologic injury of transplanted kidney.Methods Totally 23 patients after kidney transplantation were retrospectively included and divided into stable function group(group A,n=6),acute injury group(group B,n=7)and chronic dysfunction group(group C,n=10)according to the postoperative recovery of transplanted kidney function and pathologic results.Conventional ultrasound and ULM parameters were compared among groups.Pearson correlation analysis was used to explore the correlations of ULM parameters with estimated glomerular filtration rate(eGFR)and pathologic Remuzzi score.Results Significant differences of the proportion of microvascular density,mean density,blood volume,mean flow velocity and perfusion index of renal allograft were found(all P<0.01),but not of conventional ultrasound parameters,microvascular complexity level,maximal density nor maximal flow velocity was found among groups(all P>0.05).Pairwise comparison demonstrated that the proportion of microvascular density,mean velocity and perfusion index in group B were lower than those in group A(all P<0.05),while the proportion of microvascular density,mean density,blood volume,mean velocity and perfusion index in group C were lower than those in group A and B(all P<0.05).The proportion of microvascular density,mean velocity and perfusion index were weakly positively correlated with eGFR(r=0.440,0.420,0.484,all P<0.05),while the mean microvascular velocity of renal allograft was highly negatively correlated with Remuzzi score(r=-0.864,P=0.012).Conclusion ULM parameters of transplanted kidney were correlated with its function and pathologic injury.
