基于智能盆底超声构建产后盆底功能康复治疗临床管理流程:多中心研究

Constructing a clinical management process of postpartum pelvic floor functional rehabilitation therapy based on intelligent pelvic floor ultrasound:A multicenter study

摘要 目的基于智能盆底超声构建产后盆底功能康复治疗临床管理流程。方法采用多中心随机对照研究设计前瞻性将108例女性产后盆底功能障碍患者按1∶1比例随机均分为研究组与对照组。对研究组基于盆底超声肌力分级(依据肛提肌裂孔前后径变化率△AP%)进行个体化康复指导,对照组接受常规评估后的推荐治疗;治疗1个月后比较组内治疗前、后盆底超声指标及组间治疗前、后差异。结果治疗后研究组最大瓦萨尔瓦动作下膀胱后壁最低点距离(B_(Va))、尿道膀胱后角(PUA)、子宫颈最低点距离(C_(Va))、肛提肌裂孔面积(LHA)、尿道旋转角(P均<0.05),以及对照组最大瓦萨尔瓦动作下B_(Va)、PUA及LHA均较治疗前改善(P均<0.05)。治疗后研究组膀胱颈移动度(BND)、最大瓦萨尔瓦动作下C_(Va)改善均优于对照组(P均<0.05),尤以前者为著(P<0.01)。结论本研究基于智能盆底超声构建的产后盆底功能康复临床管理流程有助于推动产后盆底康复迈向标准化、精细化。 Objective To construct a clinical management process of postpartum pelvic floor functional rehabilitation therapy based on intelligent pelvic floor ultrasound.Methods A multicenter,randomized controlled study design was adopted.Totally 108 cases of postpartum female pelvic floor dysfunction were prospectively included and randomly divided into study group and control group at a ratio of 1∶1.Patients in study group underwent individualized rehabilitation guidance based on muscle strength classification of pelvic floor ultrasound according to the anteroposterior diameter change rate △AP% of the levator ani muscle hiatus,while the ones in control group were treated with recommended methods of clinicians based on routine assessment.After one month’s treatments,intra-group and inter-group comparison of pelvic floor ultrasound indicators were performed with those before treatments.Results After 1 month’s treatments,the lowest point of the bladder(B_(Va)),posterior vesicourethral angle(PUA),the lowest point of the cervix(C_(Va)),the area of the levator ani muscle hiatus(LHA)and the urethral rotation angle under the maximum Valsalva maneuver in study group all improved(all P<0.05),while B_(Va),PUA and LHA under the maximum Valsalva maneuver in control group also improved compared with those before treatments(all P<0.05).The improvement of bladder neck mobility(BND)and C_(Va)under the maximum Valsalva maneuver in study group were more obvious than those in control group(both P<0.05),especially in terms of BND(P<0.01).Conclusion The constructed clinical management process of postpartum pelvic floor functional rehabilitation therapy based on intelligent pelvic floor ultrasound was helpful to promoting the standardization and refinement of postpartum pelvic floor rehabilitation.
