脑电双频指数监测依托咪酯复合丙泊酚麻醉对股骨颈骨折手术患者麻醉深度的影响

摘要 目的:探讨脑电双频指数监测依托咪酯复合丙泊酚麻醉对股骨颈骨折手术患者麻醉深度的影响。方法:选取2023年11月至2025年8月期间收治的84例股骨颈骨折手术患者,分为对照组和观察组,各42例。对照组根据生命体征变化监测依托咪酯复合丙泊酚麻醉,观察组给予脑电双频指数监测依托咪酯复合丙泊酚麻醉。对比两组苏醒质量及术中丙泊酚总量,对比两组不同时间MAP、HR及麻醉深度,分析两组术后12 h、术后1、3、7 d疼痛程度,观察两组术前、术后1、3、7 d的认知功能,记录两组不良反应。结果:观察组拔管时间、自主呼吸恢复时间、苏醒时间及术中丙泊酚总量较对照组低(P<0.05)。T1、T5两组MAP及HR水平比较无差异(P>0.05);T2、T3及T4,观察组MAP水平高于对照组,HR水平低于对照组(P<0.05)。与术后12 h相比,术后1、3、7 d,两组疼痛程度均降低(P<0.05);但两组术后7 d疼痛程度比较有差异(P<0.05)。T2、T3、T4及T5,两组Ramsay镇静评分均上升,但观察组Ramsay镇静评分高于对照组(P<0.05)。两组术后1 d、术后3 d及术后7 d的认知功能评分均下降,但观察组认知功能评分较对照组高(P<0.05)。两组不良反应比较差异无显著差异(P>0.05)。结论:脑电双频指数监测依托咪酯复合丙泊酚麻醉,可有效把控股骨颈骨折手术患者麻醉深度。 Objective:To explore the effect of bispectral index monitoring of etomidate combined with propofol anesthesia on the depth of anesthesia in patients undergoing femoral neck fracture surgery.Methods:84 patients with femoral neck fracture who were admitted from November 2023 to August 2025 were selected and divided into control group and observation group,42 cases in each group.The control group was anesthetized with etomidate combined with propofol based on changes in vital signs,while the observation group was given etomidate combined with propofol for electroencephalogram bispectral index monitoring.The recovery quality and the dosage of propofol of the two groups were compared.The MAP,HR and anesthesia depth at different times of the two groups were compared.The pain degree of the two groups at 12 hours,1 day,3 days and 7 days after the operation was analyzed.The cognitive function of the two groups before the operation and 1 day,3 days and 7 days after the operation was observed.The adverse reactions of the two groups were recorded.Results:The observation group showed shorter intubation time,faster recovery of spontaneous breathing,earlier awakening time,and lower total intraoperative propofol dosage compared to the control group(p<0.05).No significant differences were observed in MAP and HR between groups at T1 and T5(P>0.05).At T2,T3,and T4,the observation group exhibited higher MAP levels and lower HR levels than the control group(P<0.05).Pain levels decreased in both groups at postoperative days 1,3,and 7 compared to day 12(P<0.05),but a significant difference was noted at day 7(p<0.05).The Ramsay sedation scores increased in both groups at T2,T3,T4,and T5,with the observation group showing higher scores(P<0.05).Cognitive function scores declined in both groups at postoperative days 1,3,and 7,but the observation group maintained higher scores than the control group(p<0.05).No significant differences were observed in adverse reactions between the two groups(P>0.05).Conclusion:Monitoring of etomidate combined with propofol anesthesia by bispectral index of electroencephalogram can effectively increase the depth of anesthesia for patients undergoing cervical fracture surgery.
