血清SIRT1联合铁蛋白水平对糖尿病周围神经病变的早期预测价值

The Value of Serum SIRT1 Combined with Ferritin Levels in Early Prediction of Diabetic Peripheral Neuropathy

摘要 目的:探讨血清沉默信息调节因子1(SIRT1)联合铁蛋白水平对糖尿病周围神经病变(DPN)的早期预测价值。方法:回顾性选取2022年3月至2025年3月医院收治的120例DPN患者,作为DPN组,另选取同期120例单纯2型糖尿病患者作为非DPN组,比较两组一般资料及血清SIRT1、铁蛋白水平,采用Logistic回归法分析SIRT1、铁蛋白对DPN的独立预测价值,并建立受试者工作特征(ROC)曲线分析血清SIRT1联合铁蛋白对于DPN的早期预测效能。结果:DPN组的糖尿病病程、空腹血糖、糖化血红蛋白均显著高于非DPN组(P<0.05),DPN组的血清SIRT1水平显著低于非DPN组(P<0.05),而铁蛋白水平则显著升高(P<0.05);糖尿病病程、空腹血糖、糖化血红蛋白、血清SIRT1、铁蛋白均为DPN的独立预测因素(P<0.05),其中糖尿病病程、空腹血糖、糖化血红蛋白、铁蛋白升高为危险因素,血清SIRT1升高为保护因素;ROC曲线分析显示,血清SIRT1单独预测DPN的AUC为0.552(灵敏度26.70%,特异度95.00%),铁蛋白单独预测的AUC为0.685(灵敏度60.00%,特异度73.34%)。然而,两者联合预测的AUC提高至0.833(灵敏度76.71%,特异度86.72%)。结论:血清SIRT1升高为DPN的保护因素,铁蛋白升高为危险因素,血清SIRT1联合铁蛋白可早期预测糖尿病患者周围神经病变的发生,为临床治疗提供参考意见。 Objective:To investigate the predictive value of serum silencing information regulatory factor 1(SIRT1)combined with ferritin levels in early prediction of diabetic peripheral neuropathy(DPN).Methods:A retrospective study was conducted on 120 DPN patients admitted to the Hospital from March 2022 to March 2025,forming the DPN group.Additionally,120 concurrent type 2 diabetes patients were selected as the non-DPN group.The two groups were compared in general demographics and serum SIRT1/ferritin levels.Logistic regression analysis was employed to evaluate the independent predictive value of SIRT1 and ferritin for DPN.Receiver Operating Characteristics(ROC)curves were constructed to analyze the early predictive efficacy of serum SIRT1 combined with ferritin in DPN.Results:The DPN group showed significantly higher diabetes duration,fasting blood glucose,and glycated hemoglobin levels compared to the non-DPN group(P<0.05).However,serum SIRT1 levels in the DPN group were significantly lower than those in the non-DPN group(P<0.05),while ferritin levels were markedly elevated(p<0.05).All four biomarkers—diabetes duration,fasting blood glucose,glycated hemoglobin,serum SIRT1,and ferritin—were independent predictors of DPN(P<0.05).Specifically,increased diabetes duration,fasting blood glucose,glycated hemoglobin,and ferritin were risk factors,whereas elevated serum SIRT1 served as a protective factor.ROC curve analysis demonstrated that serum SIRT1 alone showed an AUC of 0.552(sensitivity 26.70%,specificity 95.00%),while ferritin alone achieved an AUC of 0.685(sensitivity 60.00%,specificity 73.34%).The combined prediction model significantly improved to an AUC of 0.833(sensitivity 76.71%,specificity 86.72%).Conclusion:Elevated serum SIRT1 acts as a protective factor against DPN,whereas increased ferritin serves as a risk factor.The combination of serum SIRT1 and ferritin provides early prediction for diabetic peripheral neuropathy development,offering valuable clinical guidance for treatment decisions.
