摘要
目的分析儿童孤独症谱系障碍(ASD)严重程度的影响因素。方法回顾性分析2023年3月至2025年1月于广州市越秀区儿童医院就诊的284例ASD患儿的临床资料。依据儿童孤独症评定量表(CARS)评分将患儿分为轻/中度组(CARS<37分,n=122)与重度组(CARS≥37分,n=162);通过病历系统收集患儿一般情况及围生期指标,并分析病情严重程度的影响因素。结果单因素分析显示,两组家族史、父/母亲生育年龄≥35岁、母亲孕期合并症、母亲孕期服药史、窒息、新生儿黄疸史比较,差异有统计学意义(P<0.05)。logistic回归分析显示,家族史、母亲生育年龄≥35岁、母亲孕期合并症是儿童ASD病情严重程度的危险因素(P<0.05)。结论家族史、母亲生育年龄≥35岁、母亲孕期合并症是儿童ASD病情严重程度的危险因素,临床应重点关注,采取针对性预防措施,以改善患儿预后,减轻病情严重程度。
Objective To analyze the influencing factors on the severity of autism spectrum disorder(ASD)in children.Methods The clinical data of 284 children with ASD in Guangzhou Yuexiu District Children’s Hospital from March 2023 to January 2025 were retrospectively analyzed.According to the childhood autism rating scale(CARS)score,the children were divided into mild-moderate group(CARS<37 points,n=122)and severe group(CARS≥37 points,n=162).The general conditions and perinatal indicators of the children were collected through the medical record system,and the influencing factors on the severity of the disease was analyzed.Results The single-factor analysis showed that there was statistically significant difference between the two groups in terms of family history,father/mother’s age at childbirth≥35 years old,mother’s pregnancy complications,mother’s medication history during pregnancy,asphyxia,and neonatal jaundice history(P<0.05).The logistic regression analysis revealed that family history,mother’s age at childbirth≥35 years old,and mother’s pregnancy complications were risk factors for the severity of ASD in children(P<0.05).Conclusion Family history,mother’s age at childbirth≥35 years,and mother’s pregnancy complications are risk factors for the severity of ASD in children.Clinicians should pay close attention to these factors and take targeted preventive measures to improve the prognosis of the children and reduce the severity of the disease.
