摘要
目的:探索"随访网格化管理、数字信息化管理、诊疗同质化管理"(简称"网格化+信息化+同质化")模式在农村高血压病管理中的效果.方法:在紧密型医共体内,以"网格化+信息化+同质化"为特征的管理模式推进农村高血压病预防、治疗、管理相结合.结果:通过"网格化+信息化+同质化"模式管理,有效提高了高血压患者相关疾病知识的知晓率、疾病治疗率与控制率.结论:"网格化+信息化+同质化"模式为农村高血压病的基层管理提供了基础,在高血压防治以及健康管理中发挥重要作用.
Objective:To explore the effectiveness of a management model integrating"grid-based follow-up,digital information management,and homogeneous diagnosis and treatment"(abbreviated as"grid+information+homogeneous")for hypertension control in rural settings.Methods:Within a compact medical community,the model was used to combine prevention,treatment and management of hypertension in rural residents.Results:The intervention significantly improved residents'awareness of hypertension-related knowledge,treatment rate and blood-pressure control rate.Conclusion:The"grid+information+homogeneous"approach provides a feasible foundation for community-level hypertension management and plays an important role in rural cardiovascular prevention and health management.
