摘要
非动脉炎性前部缺血性视神经病变(NAION)是由睫状后短动脉灌注不足引起的视神经急性缺血,导致无痛性视力下降,甚至失明的急性视神经疾病。提高NAION患者在视盘水肿消退后的萎缩期的最终视力、视野,改善其视功能是相关领域的难题与研究热点。韦企平提出NAION急性期应从络毒论,萎缩期的关键病机为内毒清除后,受损眼络已失其正常濡养,络气不畅,络脉不荣,目系失养,主张本病萎缩期应从“扶养病络”的角度出发,充畅气络、血络,滋养目系。本文将探讨韦企平治疗NAION萎缩期的思路及临床用药经验。
Non-arteritic anterior ischemic optic neuropathy(NAION)is an acute optic neuropathy caused by hypoperfusion of the short posterior ciliary arteries leading to acute ischemia of the optic nerve and resulting in painless vision loss or even blindness.During the atrophy stage following the resolution of optic disc edema in NAION patients,improving final visual acuity,visual fields,and visual function remains a challenging and hot research topic in related fields.WEI Qiping proposed that the acute phase of NAION should be addressed from the theory of collateral toxins,while the key pathogenesis in the atrophy stage lies in the failure of the damaged eye collaterals to nourish properly after the internal toxins are cleared.This leads to obstruction of collateral Qi,lack of nourishment in the collaterals,and undernourishment of the optic system.He advocates treating NAION in the atrophy stage from the perspective of"supporting and nourishing the diseased collaterals",by promoting Qi and blood collaterals and nourishing the optic system.This article explores WEI Qiping´s treatment ideas and clinical medication experience for NAION at the atrophy stage.
