摘要
目的探讨针刺联合红外偏振光照射星状神经节(SG)对复杂性区域疼痛综合征Ⅰ型(CRPS-Ⅰ)患者血流动力学和上肢运动功能的影响。方法选取2024年4月至2025年6月在重庆医科大学附属璧山医院就诊的脑卒中后CRPS-Ⅰ患者120例作为研究对象,根据治疗方法分为对照组和研究组。对照组行针刺治疗,研究组在对照组的基础上进行红外偏振光照射SG治疗,疗程4周。比较两组神经源性炎症指标、血流动力学指标及上肢运动功能。结果倾向性评分匹配法后各组48例。治疗后,两组白细胞介素-6、肿瘤坏死因子α及神经元特异性烯醇化酶水平低于治疗前,且研究组低于对照组(P<0.05)。研究组临床疗效优于对照组(P<0.05)。研究组治疗2、4周大脑中动脉、大脑前动脉、大脑后动脉平均血流速度高于对照组(P<0.05);大脑中动脉、大脑前动脉、大脑后动脉血管阻力指数低于对照组(P<0.05)。研究组治疗2、4周血管内径低于对照组,治疗4周静脉最大血流速度高于对照组(P<0.05)。治疗后,两组Fugl-Meyer运动功能评定量表(FMA)评分高于治疗前、上肢功能评定表(DASH)评分低于治疗前,且研究组FMA评分高于对照组,DASH评分低于治疗组(P<0.05)。治疗4周,研究组疼痛程度优于对照组(P<0.05)。结论针刺联合红外偏振光照射SG可明显改善CRPS-Ⅰ患者脑血管及上肢血流动力学参数,并有效促进上肢运动功能恢复、减轻疼痛症状。该联合疗法为CRPS-Ⅰ的临床管理提供了一种安全有效的非药物干预策略。
Objective To explor the effects of acupuncture combined with infrared polarized light irradiation on stellate ganglion(SG)on the hemodynamics and upper limb motor function in patients with complex regional pain syndrome typeⅠ(CRPS-Ⅰ).Methods A total of 120 post-stroke CRPS-Ⅰpatients admitted to Bishan Hospital Affiliated to Chongqing Medical University from April 2024 to June 2025 were selected as the research subjects.According to treatment methods,they were divided into control group and study group.The control group received acupuncture treatment,while the study group was treated with infrared polarized light irradiation on SG on the basis of the control group.The treatment course was four weeks.The indicators of neurogenic inflammation,hemodynamic parameters,and upper limb motor function between two groups of patients were compared.Results After propensity score matching,there were 48 patients in each group.After treatment,the levels of interleukin-6,tumor necrosis factor-α,and neuron-specific enolase in both groups were lower than those before treatment,and the study group was lower than the control group(P<0.05).The clinical efficacy of the study group was better than that of the control group(P<0.05).The mean flow velocity of cerebral artery of the middle cerebral artery,anterior cerebral artery,and posterior cerebral artery in the study group were higher than those in the control group at two and four weeks of treatment;while the vascular resistance index of the middle cerebral artery,anterior cerebral artery,and posterior cerebral artery were lower than those of the control group(P<0.05).The vascular inner diameter of the study group at two and four weeks of treatment were lower than those of the control group,and the peak systolic velocity of the vein at four weeks of treatment was higher than that of the control group(P<0.05).After treatment,the Fugl-Meyer motor function assessment scale(FMA)scores of both groups were higher than those before treatment,while the disabilities of the arm shoulder and hand(DASH)scores were lower than those before treatment.Moreover,the FMA score of the study group was higher than that of the control group,while the DASH score was lower than that of the treatment group(P<0.05).At 4 weeks of treatment,the pain degree of the study group was better than that of the control group(P<0.05).Conclusion Acupuncture combined with infrared polarized light irradiation on SG can significantly improve cerebral vascular and upper limb hemodynamic parameters in patients with CRPS-Ⅰ,and effectively promote upper limb motor function recovery and alleviate pain symptoms.This combination therapy provides a safe and effective non pharmacological intervention strategy for the clinical management of CRPS-Ⅰ.
