摘要
目的:研究前交叉韧带(ACL)重建术后6个月移植物信号强度的相关影响因素。方法:选取2019年1月至2023年1月于深圳市第二人民医院接受ACL重建手术治疗的102例患者作为研究对象,术后6个月,使用德国Siemens 3.0T磁共振成像(MRI)扫描仪检查并测量患者移植物倾斜角、胫骨坡度、胫骨骨髓道入口位置百分比、髁间切迹宽度和胫骨入口宽度,记录股骨段T2信号、胫骨段T2信号和中段T2信号,分析不同解剖因素与移植物信号强度的相关性。结果:股骨段T2信号、胫骨段T2信号、中段T2信号均与移植物倾斜角、胫骨入口宽度呈正相关关系(P<0.05);胫骨段T2信号与胫骨坡度呈正相关关系(P<0.05);股骨段T2信号、胫骨段T2信号与髁间切迹宽度呈正相关关系(P<0.05)。影响股骨段T2信号的因素为移植物倾斜角(P<0.05);影响胫骨段T2信号的因素为移植物倾斜角及髁间切迹宽度(P<0.05);影响中段T2信号的因素为胫骨入口宽度(P<0.05);移植物倾斜角、髁间切迹宽度、胫骨坡度和胫骨入口宽度为影响移植物信号强度的因素。结论:ACL重建术后移植物信号强度与解剖因素密切相关,临床应重视此问题。
Objective To study the correlative influencing factors of graft signal intensity 6 months after anterior cruciate ligament(ACL)reconstruction.Methods A total of 102 patients who underwent ACL reconstruction at Shenzhen Second People's Hospital from January 2019 to January 2023 were selected as the research subjects.Six months after the surgery,the patients were examined and measured for graft inclination angle,tibial slope,percentage of tibial bone tunnel entry position,intercondylar notch width,and tibial entry width using a German Siemens 3.0T magnetic resonance imaging(MRI)scanner.The T2 signals of the femoral segment,tibial segment,and mid-segment were recorded,and the correlative influencing factors of different anatomical factors on graft signal intensity were analyzed.Results The T2 signals of the femoral segment,tibial segment,and mid-segment were positively correlated with the graft inclination angle and tibial entry width(P<0.05);the T2 signal of the tibial segment was positively correlated with the tibial slope(P<0.05);the T2 signals of the femoral segment and tibial segment were positively correlated with the intercondylar notch width(P<0.05).The influencing factor of the T2 signal of the femoral segment was the graft inclination angle(P<0.05);the influencing factors of the T2 signal of the tibial segment were the graft inclination angle and intercondylar notch width(P<0.05);the influencing factor of the T2 signal of the mid-segment was the tibial entry width(P<0.05);the graft inclination angle,intercondylar notch width,tibial slope and tibial entry width were the influencing factors of graft signal intensity.Conclusion The signal intensity of the graft after ACL reconstruction is closely related to anatomical factors,and this issue should be given due attention in clinical practice.
