摘要
目的探究CD138/MUM-1免疫组化双染技术在慢性子宫内膜炎(CE)病理诊断中的应用价值。方法选取2022年1月至2023年12月在合肥医科大学附属医院生殖医学中心临床诊断为不孕症、反复着床失败及复发性流产的患者子宫内膜活检样本125例,进行HE染色、CD138免疫组化单染、MUM-1免疫组化单染及CD138/MUM-1免疫组化双染检测,观察浆细胞在各组的表达情况及浆细胞数目的差异。结果以高倍镜全视野浆细胞数≥5个为CE诊断标准,各组诊断率分别为60%、82.4%、58.4%、83.2%。与HE染色相比,CD138免疫组化单染和CD138/MUM-1免疫组化双染的诊断率显著提高(P<0.001),MUM-1免疫组化单染的诊断率与HE染色相比差异不具有统计学意义(P>0.050);CD138免疫组化单染的诊断率显著高于MUM-1免疫组化单染(P<0.001),CD138/MUM-1免疫组化双染与CD138免疫组化单染的诊断率差异不显著(P>0.050),但CD138/MUM-1免疫组化双染诊断率显著优于MUM-1免疫组化单染(P<0.001)。在CE患者和非CE患者中,CD138/MUM-1免疫组化双染识别出的浆细胞数量均显著多于HE染色、CD138及MUM-1免疫组化单染(P<0.050)。结论CD138/MUM-1免疫组化双染技术在CE诊断中具有更高的检出率和准确性,同时其检测浆细胞的敏感性更强,是一种更为可靠的诊断方法。
Objective To investigate the diagnostic value of CD138/MUM-1 immunohistochemical(IHC)double staining in the pathological diagnosis of chronic endometritis(CE).Methods In total,125 endometrial biopsy samples were collected from infertile patients suffering from recurrent implantation failure or recurrent miscarriage who were diagnosed at the Reproductive Medicine Center of Zunyi Medical University Affiliated Hospital between January 2022 and December 2023.The samples were subjected to hematoxylin-eosin(HE)staining,as well as CD138 IHC single staining,MUM-1 IHC single staining,and IHC double staining.The expression patterns and differences in the number of plasma cells detected in the endometrial tissue were observed across the groups.Results The diagnostic criteria for CE were defined as the presence of at least five plasma cells across all examined high-power fields.The respective diagnostic rates for CE in each group were 60%,82.4%,58.4%,and 83.2%,respectively.The diagnostic rates for CD138 IHC single staining and CD138/MUM-1 IHC double staining were significantly higher than the diagnostic rate for HE staining(P<0.001).The diagnostic rate of MUM-1 IHC single staining did not differ significantly from that of HE staining(P>0.050).However,the diagnostic rate of CD138 IHC single staining was significantly higher than that of MUM-1 IHC single staining(P<0.001).There was no significant difference in diagnostic accuracy between CD138/MUM-1IHC double staining and CD138 IHC single staining(P>0.050);however,the diagnostic accuracy of CD138/MUM-1IHC double staining was significantly higher than that of MUM-1IHC single staining(P<0.001).In both CE and non-CE patients,the number of plasma cells identified by CD138/MUM-1 IHC double staining was significantly higher than that identified by HE staining,CD138IHC single staining,and MUM-1 IHC single staining(P<0.050).Conclusion The CD138/MUM-1 IHC double-staining technique is more accurate and offers a higher detection rate for CE.It also more sensitively detects plasma cells,making it a more reliable diagnostic method.
