高原地区健康人群膝关节X线参数测量分析

Measurement and analysis of knee joint X-ray parameters in healthy population in plateau area

摘要 目的测量高原地区健康人群膝关节的各项参数,为确定该地区膝关节正常参考值及假体设计提供更为精准的数据支持。方法于2025年8—12月对青海玉树(平均海拔4493.4 m)212名健康成人(男111名,女101名,年龄18~80岁)的膝关节正侧位X线片进行分析,测量解剖股骨远端外侧角(aLDFA)、解剖胫骨近端内侧角(aMPTA)、股胫角(FTA)、关节线夹角(JLCA)及髌骨厚度5项参数,并比较不同性别、左右侧、年龄段的差异。结果212名健康成人的aLDFA为76°~85°,平均80.23°±2.11°,aMPTA为84°~92°,平均87.68°±1.51°,FTA为166°~179°,平均172.19°±2.85°,JLCA为-1°~2°,平均0.7°±0.7°,髌骨厚度为16.9~28.7 mm,平均(20.42±2.17)mm。男性aLDFA、FTA及髌骨厚度明显大于女性(P<0.05),而男性与女性的aMPTA和JLCA比较差异无统计学意义(P>0.05);左侧JLCA明显小于右侧(P<0.05),而a LDFA、a MPTA、FTA和髌骨厚度左右侧比较差异无统计学意义(P>0.05);青年组(18~39岁)与中老年组(40岁及以上)上述参数比较差异均无统计学意义(P>0.05)。结论玉树地区健康人群的部分膝关节解剖学参数在性别与左右侧方面呈现显著差异,而在青年(18~39岁)与中老年(40岁及以上)之间未发现明显差异,但部分测量值与现有文献数据不一致。 Objective To measure various parameters of the knee joint in healthy populations in high-altitude areas,and to provide more accurate data support for the reference values of normal knee joints and prosthesis design in this region.Method This study analyzed the anteroposterior and lateral X-ray fi lms of the knee joints of 212 healthy adults(111 males and 101 females,aged 18-80 years)in Yushu,Qinghai(average altitude 4493.4 meters).Five parameters were measured:anatomical lateral distal femoral angle(aLDFA),anatomical medial proximal tibial angle(aMPTA),femorotibial angle(FTA),joint line convergence angle(JLCA),and patellar thickness.Differences among different genders,left and right sides,and age groups were compared.Result The average value of aLDFA was 80.23°±2.11°(range:76°-85°),aMPTA was 87.68°±1.51°(range:84°-92°),FTA was 172.19°±2.85°(range:166°-179°),JLCA was 0.7°±0.7°(range:-1°-2°),and patellar thickness was 20.42 mm±2.17 mm(range:16.9 mm-28.7 mm).aLDFA,FTA,and patellar thickness in males were signifi cantly greater than those in females(P<0.05),while there were no statistically signifi cant differences in aMPTA and JLCA between genders(P>0.05).The JLCA of the left side was signifi cantly smaller than that of the right side(P<0.05),and there were no statistically signifi cant differences in aLDFA,aMPTA,FTA,and patellar thickness between the left and right sides(P>0.05).There were no signifi cant differences in all parameters between the young group(18-39 years old)and the middle-aged and elderly group(over 40 years old)(P>0.05).Conclusion Certain knee joint anatomical parameters of healthy individuals in the Yushu Plateau area show signifi cant differences in terms of gender and laterality(left vs.right),whereas no obvious differences are observed among different age groups.Some measured values are inconsistent with the data reported in existing literature.The results of this study can provide an important anatomical basis for the prevention,clinical diagnosis and treatment,and customized prosthesis design of knee osteoarthritis(KOA)in this region.
