摘要
目的探讨运动负荷超声心动图(ESE)联合二维斑点追踪超声心动图(2D-STE)评估闭塞性与非闭塞性急性心肌梗死(AMI)经皮冠状动脉介入治疗(PCI)术后左心室功能的变化。方法回顾性选择2023年10月至2024年10月在嘉兴市第一医院诊治的AMI患者45例,冠脉造影显示23例为急性闭塞性心肌梗死(观察组),22例急性非闭塞性心肌梗死(对照组),组间比较一般资料、ESE及2D-STE测值,包括二尖瓣舒张早期血流峰值流速(E峰流速)、二尖瓣舒张晚期血流峰值流速(A峰流速)、二尖瓣环平均舒张早期运动速度(平均e')、E与A速度之比(E/A)、平均E与e'速度之比(平均E/e')、左心室舒张末期容积(LVEDV)、左心室收缩末期容积(LVESV)、每搏输出量(SV)、左心室射血分数(LVEF)、左心室整体纵向应变(LVGLS)、舒张储备功能(ΔE、ΔE/A、Δ平均e'、Δ平均E/e')和收缩储备功能(ΔEDV、ΔESV、ΔSV、ΔLVEF、ΔGLS)。结果观察组平静期及峰值期LVEF、LVGLS和平均e'低于对照组,而LVEDV、LVESV高于对照组,观察组峰值期A峰流速和平均E/e'高于对照组,差异均有统计学意义(均P<0.05)。观察组ΔGLS低于对照组,差异有统计学意义(P<0.05)。两组患者ΔE、ΔE/A、Δ平均e'、Δ平均E/e'比较均差异无统计学意义(均P>0.05)。结论ESE联合2D-STE有助于评估心肌梗死PCI术后患者的心功能的变化。
Objective To evaluate left ventricular function and reserve after percutaneous coronary intervention(PCI)in occlusion and non-occlusion acute myocardial infarction(MI)using exercise stress echocardiography(ESE)combined with two-dimensional speckle tracking echocardiography(2D-STE).Methods A total of 45 patients with MI treated in Jiaxing First Hospital from October 2023 to October 2024 were retrospectively enrolled.Coronary angiography revealed 23 patients with occlusive myocardial infarction(observation group)and 22 patients with non-occlusive myocardial infarction(control group).General data,ESE and 2D-STE measurements including peak early diastolic mitral flow velocity(E-wave velocity),peak late diastolic mitral flow velocity(A-wave velocity),mean early diastolic mitral annular velocity(mean e'),E/A ratio,mean E/e'ratio,left ventricular end-diastolic volume(LVEDV),left ventricular end-systolic volume(LVESV),stroke volume(SV),left ventricular ejection fraction(LVEF),left ventricular global longitudinal strain(LVGLS),diastolic reserve(ΔE,ΔE/A,Δmean e',Δmean E/e')and systolic reserve(ΔEDV,ΔESV,ΔSV,ΔLVEF,ΔGLS)were compared between groups.Results Rest and peak LVEF,LVGLS and mean e'were significantly lower,while LVEDV,LVESV,A-wave velocity and mean E/e'ratio were higher in the observation group than in the control group,peak A-wave velocity and E/e'were higher in the observation group than in the control group(all P<0.05).TheΔGLS in the observation group was lower than that in the control group(P<0.05).There were no significant differences inΔE,ΔE/A,Δmean e'andΔmean E/e'between the two groups(all P>0.05).Conclusion ESE combined with 2D-STE is helpful in evaluating changes in cardiac function in patient after PCI for AMI.
