黄斑裂孔玻璃体术后视网膜光敏度与RBFD及脉络膜厚度关系

Correlation of retinal light sensitivity with retinal blood flow density and choroidal thickness in patients with macular hole after vitreous surgery

摘要 目的探讨黄斑裂孔病人玻璃体手术后视网膜光敏感度(RLS)与视网膜血流密度(RBFD)和脉络膜厚度(SFCT)的关系。方法2021年3月—2024年6月,选取在新乡医学院第一附属医院接受玻璃体手术治疗的黄斑裂孔病人118例(118眼,均为单眼发病)为研究对象,对其临床资料进行回顾性分析。以病眼为研究组(118眼)、对侧健眼为对照组(118眼),比较两组最佳矫正视力(BCVA)、RLS、浅层视网膜血管丛(SCP)血流密度、深层视网膜血管丛(DCP)血流密度和SFCT等指标差异,并分析研究组术后各指标的变化以及RLS与RBFD和SFCT的关系。结果术前研究组BCVA(LogMAR)显著高于对照组,RLS、SCP血流密度、DCP血流密度和SFCT等指标均显著低于对照组(t=7.501~40.607,P均<0.05)。研究组术前及术后各时间点的BCVA、RLS、SCP血流密度、DCP血流密度和SFCT比较,差异均具有统计学意义(F=10.389~177.270,P均<0.05),术后1、3和6月的BCVA(LogMAR)均较术前显著降低(P均<0.05),而RLS、SCP血流密度、DCP血流密度和SFCT等均较术前显著升高(P均<0.05)。Pearson相关性分析显示,术前及术后1、3月时的RLS均与SCP血流密度、DCP血流密度和SFCT等指标呈显著正相关(r=0.437~0.567,P均<0.05),术后6月时RLS均与SCP血流密度、DCP血流密度和SFCT等指标无显著相关性(P均>0.05)。结论玻璃体手术对黄斑裂孔病人视力恢复和视网膜功能改善具有显著效果,术后3月内RLS改善与RBFD及SFCT变化均呈显著正相关。 Objective To investigate the correlation of retinal light sensitivity(RLS)with retinal blood flow density(RBFD)and subfoveal choroidal thickness(SFCT)in patients with macular hole after vitreous surgery.Methods A total of 118 patients(118 eyes with monocular lesion)with macular hole who underwent vitreous surgery in The First Affiliated Hospital of Xinxiang Medical College From March 2021 to June 2024 were enrolled as subjects,and a retrospective analysis was performed for their clinical data.The diseased eyes were included as the study group(118 eyes),and the contralateral healthy eyes were included as the control group(118 eyes).The two groups were compared in terms of best corrected visual acuity(BCVA),RLS,blood flow density in superficial capillary plexus(SCP),blood flow density in deep capillary plexus(DCP),and SFCT,and the changes in these indicators after surgery were analyzed,as well as the correlation of RLS with RBFD and SFCT.Results Before surgery,compared with the control group,the study group had a significantly higher BCVA(LogMAR)and significantly lower RLS,SCP blood flow density,DCP blood flow density,and SFCT(t=7.501-40.607,all P<0.05).The study group had significant changes in BCVA,RLS,SCP blood flow density,DCP blood flow density,and SFCT at each time point after surgery(F=10.389-177.270,all P<0.05),with a significant reduction in BCVA(LogMAR)and significant increases in RLS,SCP blood flow density,DCP blood flow density,and SFCT at 1,3,and 6 months after surgery(all P<0.05).The Pearson correlation analysis showed that RLS before surgery and at 1 and 3 months after surgery were significantly positively correlated with SCP blood flow density,DCP blood flow density,and SFCT(r=0.437-0.567,all P<0.05),while RLS at 6 months after surgery had no significant correlation with SCP blood flow density,DCP blood flow density,and SFCT(all P>0.05).Conclusion Vitreous surgery can significantly promote vision recovery and improve retinal function in patients with macular hole,and the improvement of RLS within 3 months after surgery is significantly positively correlated with the changes in both RBFD and SFCT.
