摘要
目的通过对比分析陆地与水下羊爆震伤模型的影像检查结果及临床症状,研究实验羊各器官损伤的易感性,为提升现代军事场景下的综合救治效能提供依据。方法选取普通级实验羊100只,陆地组及水下组均按距离爆炸中心点水平5、10、20 m建立羊爆震伤模型实验组(两组各距离点位均15只)和各自对照组(均5只),于爆震伤后立即按意识状态分类、监测生命体征并立即行超声检查,30~60 min内完成CT检查,对照分析CT扫描后各器官损伤数量及影像异常表现。结果无论水下组还是陆地组,实验羊距离爆炸点越近死亡数越多;无论行超声还是CT检查,组内肺的损伤数量均显著高于实性器官损伤(均P<0.01);同等爆震距离下,水下组较陆地组出现更严重的意识障碍,肺、胃肠及肝、脾、肾的损伤数量均高于后者,但差异无统计学意义(P>0.05),差异仅体现在心脏损伤及颅脑损伤(P<0.001)。结论爆震伤中,肺、胃肠道等空腔器官属于易损器官;水下环境中总体损伤器官数量均高于陆地环境中,且心脏更易损伤。
Objective Comparative analysis was conducted on the imaging examination results and clinical symptoms of terrestrial and underwater blast injury models in sheep,studying the susceptibility of various organs to injury in experimental sheep in order to provide a basis for enhancing comprehensive rescue efficacy in modern military scenarios.Methods One hundred healthy sheep were selected.The sheep blast injury models were established for both the land group and the underwater group at the distances of 5,10,and 20 meters from the explosion center.Each experiment group had 15 sheep at each distance point,and 5 sheep for each control group.Immediately after blast injury,the sheep were classified according to their consciousness status,vital signs were monitored,and ultrasound examinations were performed.CT examinations were completed within 30 to 60 minutes.The number of organ injuries and abnormal imaging findings after CT scanning were analyzed and compared.Results For both the land group and the underwater group,the closer the experimental sheep was to the explosion point,the higher the number of deaths was;regardless of ultrasound or CT examination,the number of lung injuries within the group was significantly higher than that of solid organ injuries(both P<0.01);at the same explosion distance,the underwater group exhibited more severe consciousness disorders compared to the land group,with higher numbers of injuries in lungs,gastrointestinal tract,liver,spleen,and kidneys,but the differences were not statistically significant(P>0.05),and the differences were only evident in heart injuries and cranial brain injuries(P<0.001).Conclusion In blast injuries,the hollow organs such as lungs and gastrointestinal tract are vulnerable organs.In underwater environment,the total number of injured organs is higher than that in terrestrial environment,and the heart is more prone to injury.
