摘要
目的:基于健康测量工具选择的共识标准(COSMIN)指南,系统评价卒中后认知障碍(PSCI)测量工具的方法学质量和测量属性,旨在筛选高质量评估工具。方法:计算机检索PubMed、Web of Science、MedLine、CINAHL、EMbase、Cochrane Library、Scopus、中国知网(CNKI)、万方数据库(WanFang Database)、维普数据库(VIP)、中国生物医学文献数据库中与卒中后认知障碍量表测量学属性评价有关的文献,检索时间为建库至2025年3月6日。2名研究者独立筛选文献和提取资料,并基于COSMIN指南对纳入研究进行方法学偏倚风险、测量学属性质量评价,并采用GRADE证据评价等级进行证据分级,最终形成推荐意见。结果:最终纳入22篇文献,共涉及10个卒中后认知障碍评估工具,分别为牛津认知筛查量表(OCS)、简易智能精神状态检查量表(MMSE)、脑卒中病人认知评估量表(CASP)、伯明翰认知评估量表(BCoS)、蒙特利尔认知评估量表(MoCA)、美国国立神经疾病与脑卒中研究院-加拿大脑卒中网络(NINDS-CSN)、简要认知障碍评估(BASIC)量表、轻度血管认知障碍(Mild Vascular Cognitive Impairment,MVCI)评估工具、巴罗神经研究所高级大脑功能筛查(BNIS)量表和改良认知状态电话访谈(TICS-M)。其中MMSE为A级推荐,其余的9个量表为B级推荐。结论:MMSE被评定为A级推荐,在更佳证据出现前可被暂时推荐使用,但其方法学质量有待提高,测量学属性仍需进一步验证。
Objective:Based on the consensus-based standards for the selection of health measurement instruments(COSMIN)guidelines,this study systematically evaluated the methodological quality and measurement properties of assessment tools for post-stroke cognitive impairment(PSCI),aiming to identify high-quality evaluation tools.Methods:A computerized search was conducted in PubMed,Web of Science,MedLine,CINAHL,EMbase,Cochrane Library,Scopus,CNKI,WanFang Database,VIP,and the China Biology Medicine(CBM)database for literature related to the psychometric evaluation of PSCI assessment tools.The retrieval period spanned from the establishment of the database to March 6,2025.Two researchers independently screened the literature and extracted data.Methodological risk of bias and quality of measurement properties were assessed using the COSMIN guidelines.The GRADE evidence grading system was applied to classify evidence levels,ultimately formulating recommendation levels.Results:A total of 22 studies were included,covering 10 PSCI assessment tools:the Oxford Cognitive Screen(OCS),Mini-Mental State Examination(MMSE),Cognitive Assessment Scale for Post-Stroke Patients(CASP),Birmingham Cognitive Screen(BCoS),Montreal Cognitive Assessment(MoCA),National Institute of Neurological Disorders and Stroke-Canadian Stroke Network(NINDS-CSN),Brief Assessment of Impaired Cognition(BASIC),Mild Vascular Cognitive Impairment(MVCI)assessment tool,Barrow Neurological Institute Screen for Higher Cerebral Functions(BNIS)and Telephone Interview for Cognitive Status-Modified(TICS-M).MMSE received grade A recommendation,while the remaining 9 tools were classified as grade B recommendations.Conclusions:The MMSE was classified as a grade A recommendation and could be temporarily recommended until better evidence emerges.However,its methodological quality requires improvement,and its measurement properties need further validation.
