呼出气一氧化氮检测在儿童咳嗽变异性哮喘诊疗中的应用价值分析

Analysis of the Application Value of Exhaled Nitric Oxide Testing in the Diagnosis and Treatment of Children with Cough Variant Asthma

摘要 目的:探析呼出气一氧化氮检测在儿童咳嗽变异性哮喘诊疗中的应用价值。方法:选取2023年6月至2024年6月南通市通州区人民医院儿童慢性咳嗽与哮喘门诊诊治的50例咳嗽变异性哮喘患儿(研究组)和同期在南通市通州区人民医院进行体检的50例健康儿童(健康组),对所有研究对象进行呼出气一氧化氮检测和肺功能检测。对比两组的检测结果,分析呼出气一氧化氮与肺功能指标的相关性。对研究组进行持续3个月的药物治疗,比较治疗前后的呼出气一氧化氮与肺功能指标。结果:研究组呼出气一氧化氮水平高于健康组(P<0.05)。研究组的第1秒用力呼气容积占预计值百分比(FEV_(1)%)、最大呼气流量占预计值百分比(PEF%)、第1秒用力呼气容积(FEV_(1))/用力肺活量(FVC)均低于健康组(P<0.05)。呼出气一氧化氮水平与FEV_(1)%、PEF%、FEV_(1)/FVC均呈负相关(P<0.05)。结论:咳嗽变异性哮喘患儿可出现呼出气一氧化氮水平升高及肺功能下降的情况,且呼出气一氧化氮水平与肺功能指标呈负相关,这两种指标均可用来评估患儿治疗的效果,具有较高的临床应用价值。 Objective:To analyze the application value of exhaled nitric oxide(FeNO)testing in the diagnosis and treatment of children with cough variant asthma(CVA).Methods:A total of 50 children with CVA(study group)treated in the pediatric chronic cough and asthma clinic of Tongzhou District People’s Hospital,Nantong,from June 2023 to June 2024,and 50 healthy children(healthy group)undergoing physical examinations at the same hospital during the same period were selected.All subjects underwent FeNO testing and pulmonary function tests.The test results of the two groups were compared,and the correlation between FeNO levels and pulmonary function indicators was analyzed.The study group received medication treatment for 3 consecutive months,and FeNO levels and pulmonary function indicators were compared before and after treatment.Results:The FeNO level in the study group was higher than that in the healthy group(P<0.05).The forced expiratory volume in 1 second as a percentage of predicted value(FEV_(1)%),peak expiratory flow as a percentage of predicted value(PEF%),and FEV_(1)/forced vital capacity(FVC)ratio in the study group were lower than those in the healthy group(P<0.05).FeNO levels showed a negative correlation with FEV_(1)%,PEF%,and FEV_(1)/FVC(P<0.05).Conclusion:Children with CVA exhibit elevated FeNO levels and declined pulmonary function.FeNO levels are negatively correlated with pulmonary function indicators.Both indicators can be used to evaluate the effectiveness of treatment in children,demonstrating high clinical application value.
