摘要
慢性萎缩性胃炎(CAG)是一种以胃黏膜慢性炎症、萎缩和胃腺体数量减少为特点的常见慢性胃病,具有高发病率和癌变风险,严重影响患者生活质量。灸法作为一种中医外治法,在“经脉-脏腑”相关理论指导下,常选择任脉与足阳明胃经治疗CAG且疗效显著。本研究通过梳理相关机制研究发现,灸法可通过调节胃动力、抑制胃部炎症反应、增强胃黏膜屏障功能、修复受损胃黏膜及延缓胃黏膜恶化等多途径发挥疗效,其上游机制包括穴位炎性启动,通过中枢神经系统和循环系统传递信号至靶器官并发挥作用。然而,现有研究在灸法的治疗参数、机制传导路径和临床依据方面仍存在不足。未来应加强对灸法调控CAG核心靶点的多机制协同作用的研究,探索其最佳治疗参数及量效关系,为临床治疗提供更有力的科学依据。
Chronic atrophic gastritis(CAG)is a common chronic gastric disorder characterized by persistent inflammation of gastric mucosa,atrophy,and a reduction in the number of gastric glands.It has a high incidence and carries a risk of malignant transformation,seriously affecting patients’quality of life.As an external therapy in TCM,moxibustion,guided by the‘meridian-viscera’theory,often targets the Ren Meridian and the Stomach Meridian of Foot-Yangming in treating CAG,achieving remarkable therapeutic effects.Mechanistic studies have shown that moxibustion exerts its effects through multiple pathways,including regulating gastric motility,inhibiting gastric inflammation,enhancing gastric mucosal barrier,repairing damaged gastric mucosa,and delaying mucosal deterioration.Its upstream mechanisms involve inflammatory activation at acupoints,with signals transmitted via central nervous and circulatory systems to target organs.However,current studies still have limitations regarding treatment parameters,mechanistic signaling pathways,and clinical evidence.Future research should focus on synergistic mechanisms by which moxibustion regulates key targets in CAG,explore optimal treatment parameters and dose-response relationships,and provide stronger scientific evidence for clinical application.
