摘要
目的:探究祛痒化湿方熏洗联合马应龙麝香痔疮膏治疗肛周湿疹(湿热下注型)疗效及对患者免疫功能的影响。方法:将102例肛周湿疹患者以随机数字表法分为熏洗组、痔疮膏组和联合组,每组34例。熏洗组给予祛痒化湿方熏洗治疗,痔疮膏组给予马应龙麝香痔疮膏治疗,联合组给予祛痒化湿方熏洗联合马应龙麝香痔疮膏治疗。比较三组患者中医证候积分、炎症因子水平、免疫指标水平、临床疗效及不良反应发生情况。结果:治疗后,三组患者中医证候积分均降低,且联合组更低(P<0.05);治疗后,三组肿瘤坏死因子-α(TNF-α)、白细胞介素-4(IL-4)水平均降低,联合组更低(P<0.05);治疗后,三组γ干扰素(IFN-γ)水平均升高,且联合组更高(P<0.05);治疗后,三组免疫球蛋白E(IgE)、嗜酸性粒细胞(EOS)水平均降低,且联合组更低(P<0.05);治疗后,联合组总有效率(97.06%)高于熏洗组(79.41%)、痔疮膏组(73.53%)(P<0.05),三组整体疗效比较,差异有统计学意义(P<0.05);治疗期间,三组患者均未出现明显不良反应。结论:祛痒化湿方熏洗联合马应龙麝香痔疮膏可改善肛周湿疹患者临床症状,减轻炎症反应,调节机体免疫功能,安全性较高。
Objective:To explore the therapeutic effect of Quyang Huashi Formula(祛痒化湿方)fumigation combined with Mayinglong Musk Hemorrhoid Ointment(马应龙麝香痔疮膏)in the treatment of perianal eczema(excessive damp-heat type)and its impact on the immune function of patients.Methods:A total of 102 patients with perianal eczema were randomly divided into the fumigation group,the hemorrhoid ointment group,and the combination group according to the random number table method,with 34 patients in each group.The fumigation group was treated with the Quyang Huashi Formula fumigation,the hemorrhoid ointment group was treated with Maying-long Musk Hemorrhoid Ointment,and the combination group was treated with Quyang Huashi Formula fumigation combined with Mayinglong Musk Hemorrhoid Ointment.The traditional Chinese medicine(TCM)syndrome scores,inflammatory factor levels,immune index levels,clinical efficacy,and adverse reaction occurrence were compared among the three groups of patients.Results:After treatment,the TCM syndrome scores of all three groups decreased,and the combination group had a lower score(P<0.05);after treatment,the levels of tumor necrosis factor-α(TNF-α)and interleukin-4(IL-4)in all three groups decreased,and the combination group had a lower level(P<0.05);after treatment,the levels of interferon-γ(IFN-γ)in all three groups increased,and the combination group had a higher level(P<0.05);after treatment,the levels of immunoglobulin E(IgE)and eosinophils(EOS)in all three groups decreased,and the combination group had a lower level(P<0.05);after treatment,the total effective rate of the combination group(97.06%)was higher than that of the fumigation group(79.41%)and the hemorrhoid ointment group(73.53%)(P<0.05),and the overall efficacy comparison among the three groups showed a statistically significant difference(P<0.05);during the treatment period,no obvious adverse reactions occurred in all three groups of patients.Conclusion:Quyang Huashi Formula fumigation combined with Mayinglong Musk Hemorrhoid Ointment can relieve the clinical symptoms of patients with perianal eczema,reduce inflammatory responses,regulate the body’s immune function,and has a relatively high safety.
