摘要
糖尿病心肌病心肌纤维化病位在心络,可归属于络病范畴,络病理论在其诊治过程中具有重要指导意义。该病形成过程较为缓慢,病程较久,主要病理演变过程为气阴两虚,瘀阻心络,进而由络及脉(心),符合中医学“久病入络”的基本规律。治疗当以益气养阴、化瘀通络为法,据此拟方“糖心康”,该方以生黄芪、生山药为君,葛根、丹参为臣,鬼箭羽、水蛭、全蝎为佐,在临床应用中取得良好疗效。附验案一则。
Diabetic cardiomyopathy with myocardial fibrosis is located in the heart collaterals and can be classified under the category of collateral disorders.The theory of collateral disorders plays a significant guiding role in its diagnosis and treatment.The formation process of this disease is relatively slow,with a long course of illness.The main pathological evolution process is characterized by deficiency of both qi and yin,and obstruction of the heart collaterals,which then progresses from collaterals to vessels(heart),conforming to the basic rule of“chronic diseases entering collaterals”in traditional Chinese medicine.The treatment should focus on tonifying qi and nourishing yin,resolving stasis and unblocking collaterals.Based on this,“Tangxin Kang”Formula(糖心康)is prescribed,with Huangqi(Astragali Radix)and Shanyao(Dioscoreae Rhizoma)as the sovereign medicinals,Gegen(Puerariae Lobatae Radix)and Danshen(Salvia Miltiorrhizae Radix et Rhizoma)as the ministerial medicinals,and Guijianyu(Ramulus Alatus Euonymi),Shuizhi(Hirudo)and Quanxie(Scorpio)as the adjuvant medicinals.This formula has achieved good therapeutic effects in clinical applications.A test case is attached.
