摘要
“因加而发”发病观源自《黄帝内经》,阐释疾病的发生与新邪引动故邪密切相关,后经历代医家通过临床实践对其内容进行丰富及延伸,沿用至今。高尿酸血症、痛风的发生与正气渐亏及湿、热、瘀三者关系密切,高尿酸血症之“湿”是藏于体内之故邪,后经新邪引诱,正气无力祛邪外出,化热成瘀,由浊变毒,新故合邪,发为痛风,这与“因加而发”发病理论契合,因此,临床诊疗上要抓住时机,在疾病早期及时干预,预防传变,在清热利湿化浊的治疗原则上灵活运用透、清、消、补四法,标本兼顾,做到未病先防、既病防变、瘥后防复,即攻毒于未盛之时、扶正于未衰之际、瘥后则注重调护,延缓“浊毒转化”进程。
The concept of“onset due to the combination of pathogenic factors”originated from The Yellow Emperor’s Canon of Internal Medicine,which explains that the occurrence of diseases is closely related to the stimulation of new pathogenic factors by existing ones.Over time,successive medical practitioners have enriched and expanded its content through clinical practice,and it has been used ever since.The occurrence of hyperuricemia and gout is closely related to the gradual depletion of healthy qi and the presence of dampness,heat,and stasis.The“dampness”in hyperuricemia refers to the latent pathogenic factors within the body.After being provoked by new pathogens,the body’s healthy qi is unable to expel the pathogenic factors,resulting in the generation of heat and stasis.This process involves the transfor⁃mation from turbidity to toxicity.When the new and old pathogens combine,it leads to the onset of gout.This is in line with the“onset due to the combination of pathogenic factors”theory of disease occurrence.Therefore,in clinical diagnosis and treatment,one should seize the opportunity to intervene promptly in the early stage of the disease to prevent transmission.Based on the principle of clearing heat,eliminating dampness,and transforming turbidity,the four methods of penetrating,clearing,eliminating,and tonifying should be flexibly applied to address both the symptoms and the root causes.This ensures prevention of disease before it occurs,control of disease progression after it occurs,and prevention of recurrence after recovery.It involves attacking the pathogen before it becomes too strong,supporting the body before it becomes too weak,and focusing on care and maintenance after recovery to delay the process of“turbidity-toxicity transformation”.
