摘要
溃疡性结肠炎是一种以结直肠黏膜炎性改变为特征的慢性非特异性肠道疾病,浊毒贯穿其病程始终,国医大师李佃贵提出的“浊毒伏络”理论对阐释其动态演变过程具有重要意义。肠络细微交错,有运行气血、渗灌濡养肠道之能,浊毒伏邪有潜藏渊薮、黏滞难解、蛰隐间作、久结入络的致病特点。故溃疡性结肠炎活动期为外因触发,伏邪内动,浊毒伏络,邪损络伤;缓解期病机为久病络损,伏邪内潜,浊毒隐络,肠虚络瘀。临证治疗遵“去宛陈莝”法辨证施治:活动期辛以通络,行浊散毒,去宛透邪;缓解期甘以养络,和浊缓毒,补络去宛。
Ulcerative colitis is a chronic non-specific intestinal disease characterized by inflammatory changes in the mucosa of the colon and rectum.Toxicity and turbidity run through the entire course of the disease.The theory of“turbid-toxin lurking in the collaterals”proposed by renowned traditional Chinese medicine Master LI Diangui is of great significance in explaining the dynamic evolution process of this disease.The intestinal collaterals are intricately interwoven,capable of circulating qi and blood,and nourishing and irrigating the intestines.The latent toxins and pathogenic factors have characteristics such as lurking in the abyss,being sticky and difficult to dissolve,remaining dormant and recurring,and gradually accumulating in the collaterals.Therefore,during the active stage of ulcerative colitis,it is triggered by external factors,with latent toxins moving internally,toxicity lurking in the collaterals,and damage to the collaterals and tissues;in the remission stage,the pathogenesis is characterized by long-term illness causing damage to the collaterals,latent toxins remaining internally,turbid-toxin hidden in the collaterals,and intestinal weakness with blood stasis in the collaterals.In clinical treatment,the“eliminating the stagnated pathogen”method is used for differentiation and treatment based on the syndrome.During the active stage,pungent herbs are used to unblock the collaterals,eliminate turbidity and disperse toxins,remove stagnant matter and expel pathogenic factors;in the remission stage,sweet herbs are used to nourish the meridians,harmonize turbidity and alleviate toxins,and strengthen the collaterals and eliminate stagnated pathogen.
