人工膝关节中超高分子量聚乙烯衬垫的失效分析

摘要 某人工膝关节衬垫在植入人体内5年后失效,其中超高分子量聚乙烯(UHMWPE)关节衬垫的损伤模式与关节功能实现之间存在相关性。为探究其失效机制,该研究采用微观形貌观测、氧化程度测试、小冲孔力学测试等技术对该人工膝关节衬垫进行了分析,并对可能的失效原因进行了推测。测试结果显示衬垫表面存在不同程度的磨损痕迹,并伴有明显的氧化现象,力学性能降低。基于自由基测试与红外光谱分析结果,推断辐照处理在衬垫表面诱发了更高程度的氧化降解,从而加剧了磨损率。针对上述失效原因,本研究提出了相应的改进策略,以期降低因假体失效而实施的翻修手术概率。 An artificial knee joint liner failed after 5 years of implantation in vivo,in which there was a correlation between damage to the ultrahigh molecular weight polyethylene(UHMWPE)joint liner and the realization of joint function.In the following section,a series of characterization methods,such as micromorphological observation,oxidation level test,and small punch test were used to analyze this artificial knee liner and speculate on the possible causes of failure.The results demonstrated that significant oxidation and varying wear were observed,along with a decrease in mechanical properties on the liner surface.Based on the results of free radical testing and infrared testing,it was inferred that the modified radiation caused an increased wear rate on the liner surface.In addition,effective solutions to these causes of failure have been proposed in order to reduce the number of revision surgeries due to prosthesis failure.
