摘要
目的 柔性减影(CE-Boost)技术是一种提高增强CT对比度的后处理技术。研究目的在于探讨CE-Boost技术对低剂量、低对比剂下肢CT血管成像(CTA)图像质量及诊断准确性的影响。方法 前瞻性纳入60例接受下肢CTA并随后行数字减影血管造影检查的外周动脉疾病患者。采用320排CT实施低剂量、低对比剂方案:管电压80 kV、可变螺距扫描,腘动脉触发追踪,并以体表面积为基础的个体化注射方案。获取原始CTA图像后,生成CE-Boost图像并进行对比。由两名放射科医师独立采用5分制对总体图像质量、诊断信心及远端血管显示等主观指标进行评分;同时测量并比较两组图像的CT值、噪声、信噪比(SNR)和对比噪声比(CNR)。结果 该方案平均有效辐射剂量为(0.72±0.14)mSv,对比剂用量为(48±5.24)mL。CE-Boost图像的CT值、SNR及CNR均显著高于原始图像(均P<0.05)。主观评价方面,CE-Boost图像各项评分亦显著优于原始图像(均P<0.05)。CE-Boost图像中CT值小于200 HU的不可诊断节段(2.3%比7%,P<0.05)及CT值小于350 HU的次优节段(11%比42.3%,P<0.001)均明显减少。基于血管节段的诊断性能分析显示,两组间差异无统计学意义(P>0.05)。结论 在低剂量下肢CTA中,CE-Boost技术可在保持诊断准确性的同时,显著提高客观及主观图像质量。
Objective Contrast-enhancement boost(CE-Boost)is a postprocessing technique designed to increase the degree of contrast enhancement on contrast-enhanced CT.This study investigated the impact of the CE-Boost technique on image quality and diagnostic accuracy in low-dose,low-contrast medium CT arteriography(CTA)of the lower extremities.Methods Sixty patients with peripheral arterial disease were prospectively enrolled and underwent both lower extremity CTA and digital subtraction angiography.To minimize the radiation dose and contrast medium volume,the CTA was performed on a 320-detector CT scanner using personalized body surface area-based contrast injection protocol at 80 kVp,variable helical pitch scanning,and bolus tracking with a trigger region of interest placed in the popliteal artery.CE-Boost images were generated and compared with the original CTA images.Qualitative parameters including overall image quality,diagnostic confidence,and distal vessel visualization were scored by two radiologists independently according to a five-point scale.Quantitative parameters including the CT value,noise,signal-to-noise ratio(SNR),and contrast-to-noise ratio(CNR)were compared.Results The average effective radiation dose was(0.72±0.14)mSv and contrast medium dose was(48±5.24)mL.The CE-Boost images demonstrated significantly higher CT values,SNRs,and CNRs as well as higher qualitative scores than that of the original images(all P<0.05).The CE-Boost images exhibited fewer non-diagnostic segments(CT values<200 HU,2.3%,P<0.05)and suboptimal segments(CT values<350 HU,11%,P<0.001)than the original images(7%,42.3%).The segment-based analysis indicated no significant difference in diagnostic performance between these two groups(all P>0.05).Conclusion CE-Boost technology in optimized,submillisievert lower extremity CTA improves image quality with good diagnostic accuracy.
