摘要
糖尿病足溃疡(DFU)因其迁延难愈、反复发作的特点,导致患者截肢风险和病死率显著升高。目前临床治疗DFU存在创面愈合慢、易复发等问题,中药通过多靶点、多通路、多成分、多机制的作用特点,在促进DFU修复方面显示出独特优势。PI3K/Akt信号通路作为调控细胞增殖、血管生成、炎症反应、氧化应激反应、细胞自噬和凋亡的关键通路,其调控与DFU创面难愈密切相关。本文系统总结PI3K/Akt信号通路在DFU修复中的分子机制,并着重阐述中药复方、单体及活性成分等通过干预该通路治疗DFU的研究进展,为深入探索中医药防治DFU的作用机制提供新思路,为DFU的中西医结合治疗提供理论依据,也为开发治疗DFU的新型药物提供参考。
Diabetic foot ulcer(DFU)is characterized by persistent and recurrent attacks,which leads to a significant increase in amputation risk and mortality rate.At present,clinical treatment of DFU has problems such as slow wound healing,and easy recurrence,Chinese materia medica has shown unique advantages in promoting DFU repair through action characteristics of multi target,multi pathway,multi component,and multi mechanism.PI3K/Akt signaling pathway as a key pathway regulating cell proliferation,angiogenesis,inflammatory response,oxidative stress response,autophagy,and apoptosis,is closely related to difficult healing of DFU wounds.This article systematically summarizes molecular mechanism of PI3K/Akt signaling pathway in repair of DFU,and focuses on research progress of Chinese materia medica compound prescription,monomers,and active ingredients that intervene in this pathway in the treatment of DFU,provide new ideas for exploring action mechanism of traditional Chinese medicine and pharmacy in the prevention and treatment of DFU in depth,provide theoretical basis for integration of traditional and western medicine in the treatment of DFU,also provides reference for the development of new drugs in the treatment of DFU.
