摘要
肺结节病是一种以非干酪样肉芽肿形成为特征的多系统疾病,全球发病率呈显著地域差异,且部分患者可能进展为肺纤维化。虽然西医在诊断技术和免疫抑制治疗方面取得一定进展,但长期用药不良反应及耐药复发问题仍未解决。本文探讨肺结节病的诊疗现状及在新型冠状病毒感染疫情后发病率增高的环境下纯中医治疗新思路和方法。经过多年临床实践,基于“正虚痰瘀毒结”病机理论,提出益气养阴、活血通络、化痰散结为思路的“益气活血祛痰法”,应用纯中药辨证治疗肺结节病,临床展现出独特优势。本文系统综述肺结节病的中西医诊疗进展,阐明中西医结合可减少西医治疗的不良反应、减少疾病复发及改善患者生活质量,同时纯中药辨证治疗为临床提供新的个体化治疗方案选择。
Pulmonary sarcoidosis is a multisystem disease characterized by non-caseating granuloma,global incidence rate shows significant regional difference,and some patients may progress to pulmonary fibrosis.Although western medicine has made some progress in diagnostic techniques and immunosuppressive therapy,problems of long-term adverse drug reactions and drug resistance recurrence have not been resolved.This article discusses current situation of diagnosis and treatment of pulmonary sarcoidosis and new ideas and methods of pure Chinese medicine treatment in environment of increased incidence rate after novel coronavirus infection epidemic.After years of clinical practice,based on pathogenesis theory of“deficiency of vital qi,phlegm and blood stasis,and toxin accumulation”,proposes“tonifying qi,promoting blood circulation and dispelling phlegm method”with the idea of tonifying qi and nourishing yin,promoting blood circulation for removing obstruction in collaterals,and dissipating phlegm and resolving masses,application of pure Chinese materia medica in syndrome differentiation and treatment of pulmonary sarcoidosis has demonstrated unique advantages in clinical practice.This article systematically reviews progress of traditional Chinese and western medicine diagnosis and treatment of pulmonary sarcoidosis,explains integration of traditional and western medicine can reduce adverse reaction of western treatment,reduce disease recurrence,and improve patients’quality of life,simultaneously,pure Chinese materia medica syndrome differentiation treatment provides new individualized treatment options for clinical practice.
